# Ecospirin (aspirin, Enteric-Coated)
## Overview
Ecospirin is a cyclooxygenase (COX) inhibitor that provides irreversible inhibition of platelet aggregation by blocking thromboxane A2 production. The enteric coating is designed to reduce direct gastric mucosal irritation, though it does not eliminate the risk of systemic gastrointestinal ulceration.
## Primary Indications
* Secondary prevention of myocardial infarction (MI) and ischemic stroke.
* Acute coronary syndromes (ACS).
* Management of stable/unstable angina.
* Prevention of transient ischemic attacks (TIA).
## Adult Dosing
* **Acute Coronary Syndrome:** 162–325 mg (non-enteric coated preferred for initial loading dose if possible, otherwise chewed/crushed).
* **Secondary Prevention (Maintenance):** 75–162 mg once daily.
* **Dosing Note:** Local institutional protocols may vary; maintenance doses of 81 mg are standard in many regions, while some clinicians prefer higher doses for specific cardiovascular indications.
## Pediatric Dosing
* **Kawasaki Disease (Acute Phase):** 80–100 mg/kg/day in 4 divided doses.
* **Antiplatelet:** 3–5 mg/kg/day once daily.
* **Note:** Use in children is restricted due to the high risk of Reye's syndrome following viral infections (e.g., influenza, varicella).
## Dose Adjustments

## Monitoring
* **Clinical:** Watch for signs of overt or occult bleeding (melena, hematuria, bruising).
* **Laboratory:** Monitor hemoglobin/hematocrit if long-term therapy is used.
* **Toxicity:** Monitor for tinnitus or altered mental status in elderly patients or high-dose therapy.
## Clinical Pearls
* **Absorption:** Enteric coating delays absorption; in acute MI, non-enteric coated aspirin is preferred for faster onset. If only enteric-coated is available, chew the tablet to hasten absorption.
* **Surgery:** Generally held 5–7 days prior to elective surgery depending on the procedure's bleeding risk and the indication for aspirin.
* **Compliance:** Stress the importance of daily adherence for secondary cardiovascular prevention.
