# Durveqtix (fidanacogene elaparvovec-dzkt)
## Overview
Durveqtix is an adeno-associated virus vector-based gene therapy designed to deliver a functional copy of the *F9* gene for the treatment of Hemophilia B. It is a one-time intravenous infusion that facilitates the endogenous production of Factor IX.
## Primary Indications
Treatment of adults with moderately severe to severe hemophilia B (congenital Factor IX deficiency) who currently use Factor IX prophylaxis therapy, or have current or historical life-threatening hemorrhage, or have repeated, serious spontaneous bleeding episodes.
## Adult Dosing
The recommended dose is 5 × 10¹¹ vector genomes per kilogram (vg/kg) administered as a single intravenous infusion.
## Pediatric Dosing
Safety and efficacy have not been established in patients younger than 18 years. Use in the pediatric population is not currently indicated.
## Dose Adjustments
No dose adjustments are provided for renal or hepatic impairment. Dosage is strictly calculated based on baseline body weight.
## Contraindications
There are no formal contraindications listed in the FDA label. However, it is not recommended for patients with pre-existing neutralizing antibodies to AAVRh74 variant, as determined by a validated specialized assay.
## Adverse Effects
Common adverse reactions include increased liver enzymes (ALT/AST), infusion-related reactions (nausea, fatigue, pyrexia), headache, and an increase in local inflammatory response.
## Key Drug Interactions
No formal drug-drug interaction studies have been performed. Corticosteroids are required as part of the premedication and post-infusion regimen to manage potential immune responses and liver enzyme elevations; therefore, interactions related to systemic glucocorticoid use (e.g., CYP3A4 substrates, NSAIDs, anticoagulants) must be monitored.
## Monitoring
* **Liver Enzymes:** Monitor ALT/AST levels weekly for at least the first 3 months post-infusion to detect and manage immune-mediated hepatotoxicity.
* **Factor IX Activity:** Monitor plasma Factor IX activity levels periodically to assess sustained therapeutic response.
* **Bleeding Events:** Monitor for clinical signs of hemorrhage despite treatment.
## Clinical Pearls
* **Immune Management:** Use of prophylactic corticosteroids (tapered over several weeks) is mandatory to prevent or mitigate the immune-mediated reduction in transgene expression caused by liver inflammation.
* **Baseline Testing:** Patients must be screened for anti-AAVRh74 neutralizing antibodies via a validated assay prior to administration; presence of these antibodies may render the therapy ineffective.
* **Administration:** It must be prepared and infused under the supervision of a healthcare provider experienced in the treatment of hemophilia. The infusion rate should be slowed or interrupted if infusion-related reactions occur.
