Direct oral anticoagulants (DOACs) are a class of medications used to prevent and treat blood clots. They work by inhibiting specific coagulation factors. ## Primary Indications * Prevention of stroke and systemic embolism in non-valvular atrial fibrillation (NVAF). * Treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE), and reduction in the risk of recurrent DVT and PE. * Prophylaxis of VTE in patients undergoing orthopedic surgery (hip or knee replacement). ## Adult Dosing Dosing varies by drug, indication, and patient factors (e.g., renal function, weight). Always refer to specific drug labeling or local protocols. * **Rivaroxaban:** * NVAF: 20 mg orally once daily with the evening meal. * DVT/PE treatment & prevention of recurrence: 15 mg orally once daily for 21 days, followed by 20 mg orally once daily. * Orthopedic surgery prophylaxis: 10 mg orally once daily, starting 6-8 hours after surgery. * **Apixaban:** * NVAF: 5 mg orally twice daily. * DVT/PE treatment & prevention of recurrence: 10 mg orally twice daily for 7 days, followed by 5 mg orally
