# Diloxanide Furoate
## Overview
Diloxanide furoate is a luminal amebicide used primarily to eliminate *Entamoeba histolytica* cysts from the gastrointestinal tract. It is poorly absorbed systemically and acts locally in the lumen of the bowel.
## Primary Indications
* Treatment of asymptomatic intestinal colonization (*E. histolytica* cyst passers).
* Follow-up therapy for invasive amebiasis (after treatment with a tissue amebicide like metronidazole or tinidazole) to clear persistent luminal cysts.
## Adult Dosing
* **Standard Regimen:** 500 mg orally three times daily for 10 days.
## Pediatric Dosing
* **Standard Regimen:** 20 mg/kg/day divided into three doses for 10 days.
* *Note:* Maximum dose should not exceed the adult dose (1500 mg/day).
## Dose Adjustments
* **Renal/Hepatic Impairment:** No specific dosage adjustments are established; given low systemic absorption, adjustments are rarely required. Use with caution in severe hepatic disease due to limited clinical data.
## Contraindications
* Hypersensitivity to diloxanide.
* Not indicated for the treatment of invasive amebiasis (e.g., amebic liver abscess or acute dysentery).
## Adverse Effects
* **Common:** Flatulence, nausea, vomiting, abdominal cramps, and pruritus/urticaria.
* **Rare:** Reversible elevation of liver enzymes.
## Key Drug Interactions
* Minimal systemic absorption limits significant drug-drug interactions.
* May theoretically increase the effects of other intestinal anti-infectives if used concurrently, though this is of little clinical significance.
## Monitoring
* **Clinical:** Monitor for resolution of gastrointestinal symptoms and ensure stool studies confirm the clearance of cysts after completion of therapy.
* **Safety:** Monitor for signs of hypersensitivity reactions.
## Clinical Pearls
* **Not a Systemic Agent:** Diloxanide furoate is ineffective against extra-intestinal amebiasis. Always ensure patients with symptomatic disease receive a tissue-active amebicide (e.g., metronidazole) prior to or concurrently with diloxanide.
* **Availability:** Diloxanide furoate is not available in many Western countries (e.g., USA) through standard retail pharmacies; it may require procurement via specialized travel/tropical medicine centers or the CDC Drug Service.
* **Compliance:** The 10-day duration is mandatory to ensure eradication of the cyst stage.
