# Diloxanide furoate
## Overview
Diloxanide furoate is a luminal amebicide used primarily to treat asymptomatic intestinal colonization by *Entamoeba histolytica*. It acts by killing the organism in the gut lumen but has no effect on extraintestinal (tissue) amebiasis.
## Primary Indications
- Asymptomatic intestinal *E. histolytica* infection (cyst passers).
- Follow-up treatment for symptomatic amebiasis (after treatment with a systemic tissue amebicide like metronidazole or tinidazole).
## Adult Dosing
- **Standard Regimen:** 500 mg orally three times daily for 10 days.
## Pediatric Dosing
- **Standard Regimen:** 20 mg/kg/day divided into three doses for 10 days.
- **Maximum dose:** Do not exceed 1,500 mg daily.
## Dose Adjustments
- **Renal/Hepatic Impairment:** No specific guidelines available; use with caution. Consult local infectious disease protocols or clinical pharmacists for guidance in severe hepatic impairment.
## Contraindications
- Hypersensitivity to diloxanide furoate.
## Adverse Effects
- **Common:** Flatulence, nausea, vomiting, abdominal cramps.

- **Other:** Diplopia, dizziness, and headache (rare).
## Key Drug Interactions

## Monitoring
- Monitor stools for *E. histolytica* cysts after treatment to confirm eradication (typically 2–3 weeks post-therapy).
- Monitor for resolution of gastrointestinal symptoms if patient was symptomatic.
## Clinical Pearls
- **Luminal Agent Only:** Diloxanide furoate is useless for invasive amebiasis (e.g., amebic liver abscess or acute dysentery). If invasive disease is suspected, it must be paired with tissue-active agents (metronidazole, tinidazole, or secnidazole).
- **Compliance:** Full 10-day course is essential to achieve clearance of the cyst stage.
- **Availability:** In many regions, this medication is only available via specialized import or tropical medicine clinics. Verify current local supply and prescribing labeling.
