# Dicyclomine Hydrochloride
## Overview
Dicyclomine hydrochloride is an anticholinergic and antispasmodic agent that reduces the activity of the smooth muscle in the gastrointestinal tract.
## Primary Indications
* Symptomatic relief of infantile colic.
* Treatment of irritable bowel syndrome (IBS) and other functional bowel disorders.
## Adult Dosing
* Oral: 20 mg four times daily.
* Maximum dose: 40 mg four times daily (160 mg/day).
* Intramuscular: 20 mg IM every 6 hours (if oral route not feasible).
## Pediatric Dosing
* Oral: 5 mg three to four times daily.
* The dosage may be increased to 10 mg three to four times daily if needed, based on clinical response and tolerance.
* Maximum dose for infants typically not to exceed 10 mg per dose or 40 mg/day.
* **Note:** Use in infants less than 6 months of age is generally discouraged due to potential for serious adverse effects including respiratory depression and neurological toxicity. Consult specific pediatric guidelines.
## Dose Adjustments
No specific dose adjustments are routinely recommended for renal or hepatic impairment. However, caution is advised in patients with significant impairment due to potential for accumulation and increased side effects.
## Contraindications
* Infants less than 1 month of age (due to risk of respiratory depression, central nervous system excitation, and possibly death).
* Glaucoma.
* Known hypersensitivity to dicyclomine.
* Obstructive uropathy.
* Gastrointestinal obstruction.
* Severe ulcerative colitis.
* Myasthenia gravis.
* Unstable cardiovascular status in acute hemorrhage.
## Adverse Effects
Common adverse effects include dry mouth, dizziness, blurred vision, drowsiness, nausea, constipation, and urinary retention. Less common but serious effects include confusion, hallucinations, tachycardia, and respiratory depression.
## Key Drug Interactions
* **Anticholinergic agents (e.g., antihistamines, tricyclic antidepressants, MAO inhibitors):** Additive anticholinergic effects.
* **Opioid analgesics:** Increased risk of severe constipation and paralytic ileus.
* **Potassium chloride:** Increased risk of gastrointestinal ulceration and bleeding when taken with solid dosage forms of potassium chloride.
## Monitoring
* Assess for effectiveness in reducing GI symptoms.
* Monitor for anticholinergic side effects (dry mouth, blurred vision, constipation, urinary retention, confusion).
* In infants, monitor closely for signs of adverse events, particularly respiratory depression or neurological changes.
## Clinical Pearls
* Dicyclomine is most effective when taken 30-60 minutes before meals and at bedtime.
* Due to potential for serious adverse effects, especially in infants, it is crucial to use the lowest effective dose for the shortest duration necessary.
* The potential for serious adverse events in infants under 6 months of age necessitates extreme caution and careful consideration of alternative therapies.
