# Dicyclomine Hydrochloride
## Overview
Dicyclomine hydrochloride is an anticholinergic and antispasmodic medication used to relieve smooth muscle spasms, particularly in the gastrointestinal tract. It acts by blocking the action of acetylcholine at muscarinic receptors.
## Primary Indications
* Treatment of irritable bowel syndrome (IBS) and other gastrointestinal spasms.
## Adult Dosing
* **Oral:** Typically 10 mg to 20 mg four times daily. Maximum dose is 40 mg four times daily (160 mg/day). Doses should be titrated up as tolerated.
* **Intramuscular (IM):** 80 mg IM every 4 to 6 hours as needed. This route is generally reserved for situations where oral administration is not feasible.
## Pediatric Dosing
* **Oral:** Safety and efficacy in children under 6 months of age are not established. For children 6 months and older, doses are typically lower and titrated carefully. A common starting dose is 5 mg three to four times daily. Dosing should be individualized. Consult specific pediatric guidelines.
## Dose Adjustments
No specific dose adjustments are universally recommended for renal or hepatic impairment. However, caution and careful monitoring are advised.
## Contraindications
* Hypersensitivity to dicyclomine.
* Glaucoma (angle-closure glaucoma).
* Infants less than 6 months of age due to risk of respiratory depression.
* Myasthenia gravis.
* Uncontrolled asthma.

## Adverse Effects

## Key Drug Interactions
* **Anticholinergic agents (e.g., atropine, some antihistamines, tricyclic antidepressants):** Additive anticholinergic effects, increasing the risk of adverse events.
* **MAO inhibitors:** May prolong and intensify anticholinergic effects.
* **Potassium chloride (extended-release formulations):** Increased risk of gastrointestinal ulceration when used with anticholinergics.
## Monitoring
* Monitor for signs and symptoms of anticholinergic toxicity (dry mouth, blurred vision, urinary retention, constipation, confusion, tachycardia).
* Assess for effectiveness in relieving spasms and pain.
* Monitor bowel function, especially in elderly patients or those with a history of constipation.
## Clinical Pearls
* Dicyclomine is most effective when taken before meals.
* Due to anticholinergic side effects, caution is advised in elderly patients.
* Short-term use is generally recommended. Prolonged use may mask more serious underlying conditions.
* Intramuscular administration should be used judiciously.
