# Diataps
## Overview
- **Classification**: Antidiarrheal (Loperamide), Antiflatulent (Simethicone)
- **Mechanism**: Loperamide acts on opioid receptors in the gut to slow intestinal motility. Simethicone reduces surface tension of gas bubbles in the GI tract, facilitating their passage.
## Primary Indications
1. **Acute Diarrhea** - Symptomatic relief of acute, non-specific diarrhea.
2. **Gas-related Symptoms** - Alleviates discomfort, bloating, and cramping associated with gas during diarrhea.
## Adult Dosing
### Standard Dosing
**Acute Diarrhea with Gas/Bloating**
- **Dose**: **2 mg Loperamide / 125 mg Simethicone** (one tablet/capsule)
- **Frequency**: Initially **2 tablets/capsules**, then **1 tablet/capsule** after each loose stool.
- **Route**: Oral
- **Duration**: Do not use for more than **48 hours (2 days)** without medical advice.
- **Maximum Dose**: **4 tablets/capsules (8 mg Loperamide / 500 mg Simethicone)** in 24 hours.
### Dose Adjustments
- **Renal Impairment**: No specific dose adjustment generally required.
- **Hepatic Impairment**: Use with caution; Loperamide is extensively metabolized by the liver. May need dose reduction; monitor for CNS toxicity.
- **Elderly Patients**: No specific dose adjustment, but use with caution due to increased sensitivity to adverse effects and potential for dehydration.
## Pediatric Dosing
### Neonates (0-28 days)
- **Contraindicated**: Loperamide-containing products are **contraindicated**.
- **Special Notes**: High risk of ileus, CNS depression, and respiratory depression.
### Infants (1-12 months)
- **Contraindicated**: Loperamide-containing products are **contraindicated**.
- **Special Notes**: High risk of ileus, CNS depression, and respiratory depression.
### Children (1-12 years)
- **Contraindicated**: Diataps (Loperamide/Simethicone combination) is **contraindicated in children under 12 years**.
- **Special Notes**: Risk of serious adverse effects including paralytic ileus, toxic megacolon, and CNS depression.
### Adolescents (13-18 years)
- **Dose**: Initial **2 tablets/capsules**, then **1 tablet/capsule** after each loose stool.
- **Frequency**: After each loose stool.
- **Maximum**: **4 tablets/capsules (8 mg Loperamide / 500 mg Simethicone)** in 24 hours.
- **Special Notes**: Do not use for more than 48 hours without medical advice.
## Safety Information
### Contraindications
- **Absolute**: Children **under 12 years** of age (for this combination product).
- **Absolute**: Acute dysentery (fever, blood in stool), acute ulcerative colitis, bacterial enterocolitis.
- **Absolute**: Pseudomembranous colitis associated with broad-spectrum antibiotics.
- **Absolute**: Abdominal pain without diarrhea, suspected ileus.
- **Relative**: Conditions where inhibition of peristalsis should be avoided (e.g., risk of toxic megacolon).
### Common Adverse Effects
- **Common (1-10%)**: Constipation, nausea, abdominal cramps, dizziness.
- **Serious but Rare**: Ileus, toxic megacolon (especially in IBD), angioedema.
- **Serious but Rare**: CNS depression (especially in children, or with overdose).
### Key Drug Interactions
- **P-glycoprotein Inhibitors (e.g., Quinidine, Ritonavir)**: Increase Loperamide plasma levels, increasing risk of CNS/cardiac toxicity. Monitor closely.
- **CNS Depressants (e.g., Opioids, Benzodiazepines)**: Additive CNS depression. Avoid co-administration or monitor for sedation.
- **Desmopressin**: Increases Loperamide plasma levels.
- **Cholestyramine**: May reduce Loperamide absorption. Separate administration.
## Monitoring & Follow-up
- **Before Treatment**: Assess hydration status, rule out contraindications (e.g., dysentery).
- **During Treatment**: Monitor for worsening diarrhea, abdominal pain, fever, blood in stool.
- **During Treatment**: Monitor for signs of dehydration. Stop if symptoms persist beyond **48 hours**.
- **Clinical Signs**: Watch for abdominal distension, signs of ileus (lack of bowel sounds), increasing constipation, or CNS changes (drowsiness, confusion).
## Clinical Pearls
- 💡 **Hydration is Key**: This product treats symptoms, not the cause. **Oral Rehydration Solution (ORS)** is crucial for preventing dehydration.
- 💡 **Not for Infections**: Do not use if diarrhea is bloody, black, or if accompanied by high fever. Consult a doctor immediately.
- 💡 **Stop When Better**: Discontinue use as soon as stools return to normal consistency or if constipation develops.
- 💡 **Abuse Potential**: Loperamide has opioid effects at very high doses; advise patients against exceeding recommended doses.
