# Dexamethasone in Chronic ITP
## Overview
Dexamethasone is a high-potency synthetic glucocorticoid used for the treatment of Immune Thrombocytopenia (ITP). In the context of chronic ITP (or initial management of newly diagnosed ITP), it is utilized for its rapid onset of action in modulating the immune system and inhibiting phagocytosis of antibody-coated platelets.
## Primary Indications
* First-line therapy for newly diagnosed ITP.
* Rescue/salvage therapy for chronic or refractory ITP.
## Adult Dosing
* **Standard Regimen:** 40 mg orally once daily for 4 consecutive days.
* **Cycles:** This 4-day course may be repeated every 14 to 28 days depending on hematologic response and patient tolerance.
* **Maximum:** Do not exceed 40 mg/day.
## Pediatric Dosing
* **Standard Regimen:** 0.6 mg/kg/day (maximum 40 mg/day) orally once daily for 4 consecutive days.

## Dose Adjustments
* **Hepatic Impairment:** No specific adjustment, but use with caution due to slowed metabolism.

## Monitoring
* **Baseline:** Complete blood count (CBC) with differential, fasting blood glucose, blood pressure, and screening for latent TB/Hepatitis if clinically indicated.
* **Ongoing:** Monitor platelet counts (weekly/bi-weekly during pulse phases), glucose levels, blood pressure, and signs of secondary infection or psychiatric changes.
## Clinical Pearls
* **Efficacy:** High-dose pulse dexamethasone is generally preferred over prednisone for initial treatment because it provides a more rapid rise in platelets and fewer long-term side effects due to the limited duration of therapy.
* **GI Protection:** Consider the use of a proton pump inhibitor (PPI) or H2-receptor antagonist, especially in patients with a history of peptic ulcer disease.
* **Administration:** Administer with or immediately after food to mitigate gastric irritation.
