# Dexamethasone in Chronic ITP
## Overview
Dexamethasone is a potent synthetic glucocorticoid with minimal mineralocorticoid activity. It is frequently utilized in Immune Thrombocytopenia (ITP) as "pulse" therapy to induce rapid increases in platelet counts, often preferred over continuous lower-dose prednisone due to potentially higher response rates and shorter duration of exposure.
## Primary Indications
* First-line induction therapy for newly diagnosed or relapsed/refractory symptomatic ITP.
* Preoperative management for patients with ITP requiring surgery.
## Adult Dosing
* **Standard Regimen:** 40 mg orally once daily for 4 consecutive days.
* **Cycles:** May be repeated every 14–28 days depending on initial platelet response and institutional protocols.

## Pediatric Dosing

* **Long-term/Repeated cycles:** Bone density monitoring and screening for cataracts/glaucoma.

* **Administration:** Take with food or milk to minimize gastric irritation. Administering in the morning helps mitigate insomnia.
