# Dexamethasone in Chronic Immune Thrombocytopenia (ITP)
## Overview
Dexamethasone is a potent, long-acting synthetic glucocorticoid with minimal mineralocorticoid activity. In refractory or chronic ITP, high-dose pulses are used to induce rapid, transient increases in platelet counts by suppressing macrophage-mediated platelet destruction and inhibiting autoantibody production.
## Primary Indications
Second-line therapy for chronic ITP; also utilized in acute settings to achieve rapid platelet recovery (e.g., prior to procedures).
## Adult Dosing
* **High-Dose Pulse:** 40 mg orally once daily for 4 consecutive days.
* **Cycles:** May be administered as a single 4-day cycle or repeated every 28 days for up to 6 cycles, depending on clinical response and protocol.
## Pediatric Dosing
* **High-Dose Pulse:** 40 mg/m²/day (maximum 40 mg/day) for 4 consecutive days.
* **Alternative:** Some pediatric protocols utilize 0.6 mg/kg/day (maximum 40 mg/day) for 4 days.

## Dose Adjustments
* **Hepatic Impairment:** Clearance may be reduced. Monitor closely for increased systemic exposure.
* **Elderly:** Use lowest effective dose; monitor for increased risk of psychiatric side effects, hyperglycemia, and bone density loss.
## Contraindications
* Systemic fungal infections.
* Known hypersensitivity to dexamethasone.
* Administration of live or live-attenuated vaccines during high-dose therapy.
## Adverse Effects
* **Common:** Hyperglycemia, insomnia, mood lability (anxiety, depression, euphoria), increased appetite, fluid retention/edema, and dyspepsia.
* **Serious:** Increased infection risk, hypertension, peptic ulcer disease, myopathy, avascular necrosis (typically with repeated cycles), and adrenal insufficiency (if abruptly discontinued after prolonged use).
## Key Drug Interactions
* **CYP3A4 Inducers (e.g., phenytoin, carbamazepine, rifampin):** May decrease dexamethasone efficacy.
* **Anticoagulants/Antiplatelets:** Increased risk of gastrointestinal bleeding.
* **NSAIDs:** Increased risk of gastric ulceration.
* **Antidiabetic Agents:** Dexamethasone may antagonize glycemic control; dose adjustments may be required.
## Monitoring
* **Platelet Counts:** Baseline and at least weekly during the peak/trough cycle.
* **Metabolic:** Blood glucose (fasting or random), blood pressure.
* **Clinical:** Signs of infection, fluid retention, and psychiatric status.
## Clinical Pearls
* **Rapid Response:** If a patient does not respond by day 7–10, additional cycles are unlikely to yield a durable response.
* **Gastroprotection:** Consider PPI or H2-receptor antagonist therapy in patients at high risk for GI ulceration.
* **Tapering:** While the 4-day pulse usually does not require a formal taper, clinical judgment should be used if the patient has received prior prolonged steroid exposure to avoid withdrawal symptoms.
* **Sleep:** Advise patients to take the dose in the morning to minimize insomnia.
