# Dexamethasone in Chronic ITP
## Overview
Dexamethasone is a potent synthetic glucocorticoid with minimal mineralocorticoid activity. In Immune Thrombocytopenia (ITP), it is utilized for its rapid immunomodulatory effects, including suppression of macrophage phagocytosis of antibody-coated platelets and inhibition of autoantibody production. It is increasingly preferred over prednisone for initial therapy due to faster response times and potential for improved durability.
## Primary Indications
First-line treatment of newly diagnosed ITP and management of refractory/chronic ITP flares.
## Adult Dosing
**Standard Regimen:** 40 mg orally once daily for 4 consecutive days.
**Repeat cycles:** May be repeated every 14–28 days depending on platelet response and clinical judgment. Up to 6 cycles may be administered before declaring treatment failure.
## Pediatric Dosing
**Standard Regimen:** 40 mg/m²/day (max 40 mg/dose) orally for 4 days.
Dosing should be verified against institutional protocols or current hematology guidelines (e.g., ASH guidelines), as pediatric dosing can vary significantly based on institutional preference.
## Dose Adjustments
* **Hepatic Impairment:** No specific adjustment; however, monitor closely as clearance may be altered.
* **Renal Impairment:** No adjustment required.
* **Concomitant medications:** Doses may require adjustment when co-administered with strong CYP3A4 inducers or inhibitors.
## Contraindications
* Systemic fungal infections.
* Known hypersensitivity to corticosteroids.
* Recent live virus vaccination (administration is often held during steroid therapy).
## Adverse Effects
* **Common:** Insomnia, mood disturbances (euphoria or depression), hyperglycemia, increased appetite, fluid retention/edema, and epigastric discomfort.
* **Serious:** Increased risk of infection, gastrointestinal ulceration/perforation, hypertension, osteoporosis (long-term), and avascular necrosis.
## Key Drug Interactions
* **CYP3A4 Inducers (e.g., Rifampin, Phenytoin, Carbamazepine):** Decrease dexamethasone efficacy.
* **CYP3A4 Inhibitors (e.g., Ritonavir, Clarithromycin, Ketoconazole):** Increase dexamethasone serum concentrations and risk of toxicity.
* **NSAIDs/Aspirin:** Increased risk of peptic ulcer disease and GI bleeding.
* **Antidiabetic agents:** Dexamethasone induces hyperglycemia, potentially requiring increased doses of insulin or oral hypoglycemics.
## Monitoring
* **Baseline:** Complete blood count (CBC) with differential, fasting blood glucose, blood pressure, and mental health screening.
* **Ongoing:** Monitor platelet counts (weekly or as clinically indicated), glucose levels (especially in patients with diabetes), and signs of infection or GI distress.
## Clinical Pearls
* **Administration:** Administer with food to mitigate GI irritation.
* **Dexamethasone vs. Prednisone:** Dexamethasone is often favored in ITP for its shorter treatment course and lower risk of traditional "steroid face" and other chronic side effects associated with prolonged prednisone tapers.
* **Steroid Response:** If no platelet response occurs after 6 cycles of dexamethasone, the patient is generally considered steroid-refractory, and second-line therapies (e.g., TPO-RAs, Rituximab, or Splenectomy) should be considered.
* **Consultation:** Always refer to specific institutional hematology guidelines, as the number and frequency of pulse doses may vary based on local practice.
