# Cystone Forte
## Overview
Cystone Forte is an Ayurvedic proprietary formulation manufactured by Himalaya Wellness. It is a concentrated version of the standard Cystone tablet, used primarily for urolithiasis and urinary tract infections. It acts as a lithotriptic, diuretic, and urinary antiseptic.
## Primary Indications
* Management and prevention of urolithiasis (kidney stones).
* Crystalluria.
* Adjuvant therapy in urinary tract infections (UTIs).
* Management of burning micturition (dysuria).
## Adult Dosing
* **Renal Calculi:** 1 tablet twice daily.
* **Urinary Tract Infections:** 1 tablet twice daily.
* **Duration:** Typically continued until the stone is expelled or symptoms subside, as directed by a healthcare provider.
## Pediatric Dosing
Safety and efficacy data for pediatric populations are limited. **Do not use in children without specialized Ayurvedic consultation and physician oversight.** There is no standardized pediatric dose for the "Forte" formulation.
## Dose Adjustments
* **Renal/Hepatic Impairment:** No established guidelines available. Use with caution in patients with severe renal insufficiency due to the potential for hyperkalemia or unknown electrolyte interactions inherent in herbal formulations.
## Contraindications
* Hypersensitivity to any components of the proprietary blend.
* Large stones (>10mm) requiring surgical intervention.
* Patients with acute urinary obstruction or severe hydronephrosis.
## Adverse Effects
* Generally considered well-tolerated.
* Gastrointestinal upset, nausea, or abdominal discomfort in sensitive individuals.
* Potential for allergic cutaneous reactions.
## Key Drug Interactions
* **Diuretics:** May have an additive effect with thiazide or loop diuretics. Monitor for electrolyte fluctuations.
* **Antibiotics:** May theoretically alter the absorption or efficacy of co-administered antibiotics (e.g., fluoroquinolones, nitrofurantoin). Administer at least 2 hours apart.
* **Variable Standardization:** As an herbal product, batch-to-batch consistency may vary, and active ingredient concentration is not regulated to the same standard as synthetic pharmaceuticals.
## Monitoring
* Monitor urine pH and serial imaging (ultrasound/CT) if used for lithiasis treatment.
* Monitor clinical symptoms (pain, frequency, urgency) to assess therapeutic failure.
* Assess renal function (creatinine, GFR) in patients with existing CKD.
## Clinical Pearls
* **Not a Replacement for Surgery:** Cystone Forte is not intended for the management of ureteral obstruction or large stones that require medical emergency intervention.
* **Hydration:** Efficacy is highly dependent on achieving adequate diuresis; counsel patients to maintain high oral fluid intake.
* **Lack of Clinical Data:** Large-scale, double-blind, placebo-controlled trials are limited. Standard evidence-based treatments for kidney stones remain the primary medical standard of care.
* **Regulatory Note:** This product is classified as a dietary supplement/herbal product in many jurisdictions, not a regulated drug. Quality control guidelines differ significantly from USP-standard pharmaceutical products.
