# Cystone Forte
## Overview
Cystone Forte is an Ayurvedic proprietary herbal formulation marketed primarily by Himalaya Wellness. It is a concentrated version of the standard Cystone tablet. As a botanical product, it is not FDA-approved, lacks standardized pharmacokinetics, and clinical evidence for efficacy in urolithiasis is limited and heterogeneous.
## Primary Indications
Used as an adjuvant therapy for the management of urolithiasis (kidney stones), crystalluria, and as a supportive treatment for urinary tract infections (UTIs) or burning micturition.
## Adult Dosing
Standard protocol is typically 1 tablet twice daily. Duration of therapy is dependent on clinical response and stone clearance, often ranging from 3 to 6 months. There is no universally established maximum dose; follow manufacturer-specific guidelines as local protocols vary.
## Pediatric Dosing
There is no established, evidence-based pediatric dose. Use is generally discouraged in children without direct supervision by a physician experienced in integrative or Ayurvedic medicine.
## Dose Adjustments
No standardized data exists for hepatic or renal impairment adjustments. Caution is advised in patients with severe renal insufficiency, as the impact of the botanical components on glomerular filtration or electrolyte handling is unknown.
## Contraindications
Hypersensitivity to any of the ingredients (commonly includes *Didymocarpus pedicellata*, *Saxifraga ligulata*, *Rubia cordifolia*, etc.). Not recommended for patients with obstruction of the urinary tract where surgical intervention is indicated.
## Adverse Effects
Generally considered safe, but gastrointestinal upset (nausea, abdominal discomfort) may occur. Due to the lack of stringent regulatory oversight, potential for contamination (e.g., heavy metals) exists depending on the specific batch and manufacturing standards.
## Key Drug Interactions
The potential for interaction with conventional medications is high due to lack of comprehensive pharmacokinetic data. Specifically, monitor for interactions with:
* **Diuretics:** Potential additive effects.
* **Antihypertensives:** Possible modulation of blood pressure.
* **Lithium:** Botanical constituents may affect renal excretion.
* **CYP450 Metabolized Drugs:** Herbs may induce or inhibit cytochrome P450 enzymes.
## Monitoring
Monitor for symptomatic relief of urinary symptoms. If symptoms of urinary obstruction, severe pain, or fever persist or worsen, patients should seek immediate conventional medical evaluation. Periodic monitoring of electrolyte panels and renal function is prudent for long-term use.
## Clinical Pearls
* **Not a replacement:** Cystone Forte should not replace standard evidence-based treatments for kidney stones (e.g., medical expulsive therapy, lithotripsy, or antibiotics for UTI).
* **Hydration:** Efficacy relies heavily on adequate fluid intake; patient compliance with hydration is more critical than the medication itself.
* **Regulation:** As an "herbal supplement," manufacturing consistency and purity are not held to the same global standards as pharmaceutical medications.
