# Cystone Forte
## Overview
Cystone Forte is a proprietary polyherbal Ayurvedic formulation marketed primarily by Himalaya Wellness. It is designed to act as a lithotriptic (stone-dissolving) and diuretic agent. Its mechanism is attributed to its constituent herbs (e.g., *Didymocarpus pedicellata*, *Saxifraga ligulata*) which purportedly inhibit crystal aggregation and promote the clearance of urinary calculi. **Note:** There is a lack of rigorous, large-scale, randomized controlled trials (RCTs) confirming its efficacy compared to standard pharmacological interventions.
## Primary Indications
* Prophylaxis and treatment of urolithiasis (kidney stones).
* Crystalluria.
* Adjunctive therapy for urinary tract infections (UTIs) and burning micturition.
## Adult Dosing
* **Renal Calculi:** 1 tablet twice daily.
* **Recurrence Prophylaxis:** 1 tablet daily for 4–6 months.
* **Urinary Tract Infection:** 1 tablet twice daily for 7–14 days.
* *Note:* There is no widely accepted "maximum" dose established by clinical pharmacopoeia standards; follow the manufacturer's product insert.
## Pediatric Dosing
* **Pediatric use is not standardized.** Consult a pediatrician before use. In some clinical practices, doses are scaled based on weight/age, but due to insufficient safety data in children, it should be used with extreme caution.
## Dose Adjustments
* **Renal Impairment:** No specific criteria exist. Cystone Forte contains potassium and various botanical compounds that may accumulate in severe renal failure. Use with caution in patients with stage 4 or 5 CKD.
* **Hepatic Impairment:** No data available.
## Contraindications
* Known hypersensitivity to any of the herbal constituents.
* Large obstructive calculi requiring surgical or lithotripsy intervention.
* Acute renal failure or anuria.
## Adverse Effects
* Generally well-tolerated.
* Gastrointestinal: Nausea, abdominal discomfort.
* Hypersensitivity: Skin rash or pruritus.
## Key Drug Interactions
* **Diuretics:** Potential additive effect; monitor for dehydration or electrolyte imbalances.
* **Anticoagulants/Antiplatelets:** Some herbal constituents may theoretically increase bleeding risk at high doses.
* **General:** Because it is a polyherbal product, the potential for enzyme induction/inhibition (CYP450) is largely unstudied.
## Monitoring
* Monitor urinary pH and crystal frequency.
* Assess serial imaging (ultrasound/CT) to track stone size progression.
* Monitor serum electrolytes (especially potassium) if used long-term in patients with renal vulnerability.
## Clinical Pearls
* **Not a replacement for surgery:** Do not delay necessary surgical intervention for large or obstructive stones in favor of herbal therapy.
* **Hydration:** Maintain high fluid intake (2–3 liters daily) to maximize efficacy.
* **Product Standardization:** As an Ayurvedic formulation, batch-to-batch consistency and the presence of heavy metals or contaminants can vary by region and regulatory oversight.
* **Evidence Status:** Regarded more as a supplement/adjunct rather than a first-line clinical medication in major nephrology guidelines.
