# Clotrimazole 1% Otic Solution
## Overview
Clotrimazole is a synthetic imidazole antifungal agent that works by inhibiting the synthesis of ergosterol, a vital component of fungal cell membranes. This leads to increased cell membrane permeability and cell death.
## Primary Indications
* Fungal infections of the ear canal (otomycosis), commonly caused by *Aspergillus* or *Candida* species.
## Adult Dosing
* Instill 2-3 drops into the affected ear(s) 3-4 times daily. The duration of treatment is typically 2-4 weeks, but may be extended based on clinical response.
## Pediatric Dosing
* Data for pediatric use is limited. Dosage and administration should be individualized by the prescribing clinician, considering the child's weight and the severity of the infection. Generally, the adult dose may be used in older children, but caution is advised in younger children.
## Dose Adjustments
* No dose adjustments are typically required for renal or hepatic impairment, as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to clotrimazole or any component of the formulation.
* Use with caution in patients with perforated tympanic membrane or middle ear infections, as systemic absorption could potentially increase.
## Adverse Effects
* Local irritation, burning, stinging, itching, redness, and peeling of the skin in the ear canal.
* Rarely, allergic reactions.
## Key Drug Interactions
* No significant drug interactions are expected due to minimal systemic absorption.
## Monitoring
* Monitor for signs of local irritation or allergic reaction.
* Assess for improvement in symptoms of fungal otitis externa.
## Clinical Pearls
* Ensure the ear canal is clean and dry before applying the drops.
* Advise patients to avoid getting water in the ear during treatment.
* Treatment duration should be sufficient to eradicate the fungal infection, even if symptoms improve earlier.
