# Clotrimazole Drops
## Overview
Clotrimazole is a synthetic imidazole antifungal agent that inhibits the synthesis of ergosterol, a vital component of fungal cell membranes.
## Primary Indications
* **Otomycosis:** Fungal infections of the external ear canal.
* **Onychomycosis:** Fungal infections of the nails, though topical solutions are generally less effective than oral agents for severe infections.
## Adult Dosing
* **Otomycosis:** Apply 2-3 drops into the affected ear(s) 3-4 times daily. Duration of treatment is typically 7-14 days.
* **Onychomycosis:** Apply to affected nail(s) 1-2 times daily. Treatment duration is prolonged, often months, and efficacy is variable.
## Pediatric Dosing
* **Otomycosis:** Apply 2-3 drops into the affected ear(s) 3-4 times daily. Recommended for children over 3 years of age. Safety and efficacy in younger children are not established.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment with topical clotrimazole.
## Contraindications
* Hypersensitivity to clotrimazole or any component of the formulation.
## Adverse Effects
* **Local:** Burning, itching, redness, stinging, peeling, rash, blistering at the application site.
* **Systemic (rare with topical):** Allergic reactions.
## Key Drug Interactions
No significant drug interactions are expected with topical clotrimazole.
## Monitoring
* Monitor for signs of local irritation or allergic reaction.
* Assess for improvement of fungal infection symptoms.
## Clinical Pearls
* Ensure the ear canal is clean and dry before application for otomycosis.
* For onychomycosis, it is crucial to trim infected nails and apply the medication consistently to the nail bed and surrounding skin.
* Topical clotrimazole may not be sufficient for extensive or deep fungal infections.
