# Clotrimazole Drops
## Overview
Clotrimazole is a synthetic broad-spectrum antifungal agent. It works by inhibiting the synthesis of ergosterol, a vital component of fungal cell membranes, leading to increased permeability and cell death.
## Primary Indications
* Otic infections due to susceptible fungi (e.g., *Candida albicans*, *Aspergillus niger*).
* Topical treatment of superficial dermatophytoses (tinea pedis, tinea cruris, tinea corporis) and cutaneous candidiasis.
## Adult Dosing
* **Otic:** Apply 3-4 drops into the affected ear(s) 3-4 times daily for 7-14 days.
* **Topical (Cream/Solution):** Apply a thin layer to affected area(s) 2-3 times daily and rub gently. Treatment duration varies but typically 2-4 weeks.
## Pediatric Dosing
* **Otic:** Safety and efficacy in pediatric patients have not been established for otic use. Consult local guidelines or specialist.
* **Topical (Cream/Solution):** Apply the same as adult dosing.
## Dose Adjustments
No dose adjustments are typically required for renal or hepatic impairment with topical or otic formulations.
## Contraindications
* Hypersensitivity to clotrimazole or any component of the formulation.
## Adverse Effects
* **Otic:** Local irritation, burning, stinging, itching, redness, blistering.
* **Topical:** Skin irritation, burning, stinging, redness, peeling, rash, itching, hives.
## Key Drug Interactions
None typically significant with topical or otic formulations due to minimal systemic absorption.
## Monitoring

* Assess for clinical improvement of fungal infection.
## Clinical Pearls
* Ensure the ear canal is clean and dry before administering otic drops.
* Avoid contact with eyes.
* For topical use, apply to clean, dry skin. Continue treatment for the recommended duration even if symptoms improve earlier to ensure complete eradication of the fungus.
* If symptoms do not improve after a specified treatment period, re-evaluate diagnosis and consider alternative therapy.
