# Clotrimazole Drops
## Overview
Clotrimazole is an imidazole antifungal agent available as a topical solution (drops) and as an oral suspension (drops) for mucosal Candida infections. Concentrations vary: topical drops 1% (10 mg/mL), oral drops also 10 mg/mL. Verify product labeling.
## Primary Indications
- Oropharyngeal candidiasis (thrush) – oral drops
- Cutaneous fungal infections (tinea, intertrigo) – topical drops
- External otitis (fungal) – topical ear drops (use specific otic formulation if available)
## Adult Dosing
- **Oral drops (thrush)**: 1 mL (10 mg) applied to affected area four times daily for 7–14 days. Swish and swallow or hold in mouth as directed.
- **Topical drops**: Apply thin layer to clean, dry skin twice daily (morning and evening) for 2–4 weeks.
## Pediatric Dosing
- **Oral drops (thrush) – infants/children**: 0.5–1 mL (5–10 mg) into each side of mouth, 4 times daily after meals. Continue at least 48 hours after symptoms resolve. For neonates, 4–6 drops (0.2–0.3 mL) per side. Dosing may vary by local protocol.
- **Topical drops**: Same as adult – apply to affected area twice daily. Safety in children <2 years not established for certain indications; use conservatively.
## Dose Adjustments
- No systemic dose adjustments needed for topical/oral mucosal use. Minimal absorption.
## Contraindications
- Hypersensitivity to clotrimazole or any component.
- Not for ophthalmic use.
## Adverse Effects
- Local irritation, burning, stinging, itching, redness.
- Oral: nausea, unpleasant taste, vomiting (rare).
- Contact dermatitis (prolonged use).
## Key Drug Interactions
- Minimal systemic interactions. Topical use: none significant. Oral drops may delay absorption of other oral medications if applied concurrently.
## Monitoring
- Clinical response (resolution of lesions, symptoms). No routine lab monitoring required.
## Clinical Pearls
- Shake bottle well before use.
- Oral drops can be diluted with water or milk for infants.
- Continue treatment for at least 2 weeks for dermatophyte infections; longer for interdigital areas.
- Avoid occlusion unless directed.
