# Clobazam (Oral)
## Overview
Clobazam is a benzodiazepine derivative with anticonvulsant properties. It is thought to exert its effects by enhancing gamma-aminobutyric acid (GABA)ergic neurotransmission.
## Primary Indications
Adjunctive therapy for seizures associated with Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older.
## Adult Dosing
* **Initial Dose:** 5 mg orally twice daily.
* **Titration:** Increase dose gradually, if needed, at weekly intervals.
* **Usual Maintenance Dose:** 10 mg to 20 mg orally twice daily.
* **Maximum Dose:** 40 mg orally daily.
## Pediatric Dosing (2 years and older)
* **Initial Dose:** 2.5 mg to 5 mg orally twice daily.
* **Titration:** Increase dose gradually, if needed, at weekly intervals.
* **Usual Maintenance Dose:** 5 mg to 10 mg orally twice daily.
* **Maximum Dose:** 20 mg orally daily.
Dosing should be individualized based on clinical response and tolerability. Lower initiation doses and slower titration may be necessary for certain pediatric populations.
## Dose Adjustments
* **Hepatic Impairment:** Use with caution. Consider lower doses and slower titration. Data is limited.
* **Renal Impairment:** No specific dose adjustments are established, but caution is advised, especially in severe impairment.
## Contraindications
* Known hypersensitivity to clobazam or other benzodiazepines.
* Severe hepatic impairment.
## Adverse Effects
* **Common:** Somnolence, drooling, constipation, decreased appetite, fatigue, aggression, vomiting, upper respiratory tract infection.
* **Serious:** Serious skin reactions (e.g., Stevens-Johnson syndrome), suicidal behavior and ideation, withdrawal symptoms, respiratory depression (especially when combined with other CNS depressants).
## Key Drug Interactions
* **CNS Depressants (e.g., alcohol, opioids, other sedatives):** Increased sedation and respiratory depression. Avoid concurrent use or use with extreme caution.
* **CYP3A4 inhibitors/inducers:** May alter clobazam concentrations. Monitor for efficacy and toxicity.
* **CNS Stimulants:** May reduce the effectiveness of stimulants.
## Monitoring
* Monitor for seizure frequency and severity.
* Assess for signs of somnolence, sedation, and behavioral changes.
* Watch for signs of withdrawal if the drug is discontinued abruptly.
* Monitor for suicidal behavior or ideation.
* Evaluate for serious skin reactions.
## Clinical Pearls
* Clobazam is a Schedule IV controlled substance.
* Gradual titration and/or dose reduction may be needed to minimize adverse effects.
* Discontinue gradually to avoid withdrawal symptoms.
* Tapering dose over several weeks is recommended upon discontinuation.
* Be aware of the potential for drug tolerance with long-term use.
