# Ciproxin (Ciprofloxacin)
## Overview
Ciprofloxacin is a second-generation fluoroquinolone antibiotic. It exerts bactericidal activity by inhibiting DNA gyrase and topoisomerase IV, preventing bacterial DNA replication. It has potent activity against Gram-negative organisms (*Pseudomonas aeruginosa*) and moderate coverage of atypical bacteria and some Gram-positive cocci.
## Primary Indications
* Urinary tract infections (UTIs)
* Respiratory tract infections (limited due to poor *S. pneumoniae* coverage)
* Infectious diarrhea
* Bone, joint, and skin/skin-structure infections
* Post-exposure prophylaxis for inhalation anthrax
## Adult Dosing
Standard oral/IV dosing ranges from 250 mg to 750 mg every 12 hours depending on severity and site.
* **Uncomplicated UTI:** 250 mg every 12 hours for 3 days.
* **Complicated UTI/Pyelonephritis:** 500 mg every 12 hours for 7–14 days.
* **Severe Systemic Infections:** 400 mg IV every 8–12 hours.
* **Maximum Dose:** Generally 1.5 g per day.
## Pediatric Dosing
Use is generally reserved for situations where safer alternatives are unavailable (e.g., *Pseudomonas* or Anthrax).
* **General/Infectious:** 20–30 mg/kg/day divided every 12 hours (Max 1.5 g/day).
* **Cystic Fibrosis (Pseudomonas):** Up to 40 mg/kg/day divided every 12 hours.
* **Note:** Use is governed by institutional policy and risk-benefit analysis due to potential arthropathy.
## Dose Adjustments
* **Renal Impairment:** Requires adjustment if CrCl ≤ 50 mL/min.
* CrCl 30–50 mL/min: 250–500 mg every 12 hours.
* CrCl <30 mL/min: 250–500 mg every 24 hours.
* **Hepatic Impairment:** No specific adjustment required; use with caution in severe disease.
## Contraindications
* Hypersensitivity to fluoroquinolones.
* Concurrent administration with tizanidine.
* History of myasthenia gravis (may exacerbate muscle weakness).
## Adverse Effects
* **Common:** Nausea, diarrhea, elevated liver enzymes, dizziness.
* **Serious (Black Box Warnings):** Tendonitis and tendon rupture (risk increased in >60 years, concurrent corticosteroids, or transplant recipients), peripheral neuropathy, and CNS effects (seizures, hallucinations).
* **Cardiac:** QTc prolongation.
* **Metabolic:** Dysglycemia (hypo- or hyperglycemia), especially in elderly diabetic patients.
## Key Drug Interactions
* **Cationic compounds:** Antacids (Mg/Al), calcium, iron, zinc, and sucralfate significantly decrease oral bioavailability. Administer Cipro 2 hours before or 6 hours after these agents.
* **CYP450 Inhibitors:** Potentiates effect of Warfarin (monitor INR) and Theophylline.
* **QT Prolonging Agents:** Increased risk of arrhythmias when combined with amiodarone, ondansetron, or antipsychotics.
## Monitoring
* **Renal Function:** Monitor CrCl, especially in geriatric populations.
* **Blood Glucose:** Monitor in patients with diabetes.
* **Cardiac:** ECG if high-risk for QTc prolongation.
* **Clinical:** Assess for signs of tendon pain/rupture or neurotoxicity.
## Clinical Pearls
* **Poor activity:** Ciprofloxacin is ineffective against MRSA and has poor activity against *Streptococcus pneumoniae*.
* **Hydration:** Maintain adequate fluid intake to prevent crystalluria.
* **Avoidance:** Avoid in uncomplicated infections (acute sinusitis, bronchitis) if safer options exist due to "collateral damage" (e.g., *C. difficile* infection, resistance).
* **Local resistance:** Always defer to institutional antibiograms, as *E. coli* resistance to ciprofloxacin is increasingly common.
