# Chymosin forte
## Overview
Chymosin forte is a commercial preparation containing chymopapain, a proteolytic enzyme derived from papaya latex. It is primarily used as an adjunct in the management of intervertebral disc herniation.
## Primary Indications
* Adjunct in the conservative management of symptomatic herniated nucleus pulposus (HNP) of the lumbar spine in patients who have failed conservative therapy.
## Adult Dosing
* **Intradiscal Injection:** The dose is determined by the surgeon based on the size and location of the herniated disc. Typically, 2000-4000 NF units (National Formulary units) are injected into the nucleus pulposus via a transcutaneous needle. The maximum recommended dose is 7500 NF units per patient per episode. Injections may be repeated at intervals of 7 days or longer if clinically indicated, not to exceed 2 treatments.
## Pediatric Dosing
* Safety and efficacy in pediatric patients have not been established. Use is not recommended.
## Dose Adjustments
* No specific dose adjustments are required for renal or hepatic impairment.
## Contraindications
* Known hypersensitivity to chymopapain or papaya derivatives.
* Infection at the injection site.
* Spinal cord compression.
* Abscess or tumor of the spine.
* Active spondylolisthesis or spinal stenosis.
* Pregnancy.
## Adverse Effects
* **Common:** Back pain at the injection site, muscle spasm, local edema.
* **Serious:** Anaphylaxis (potentially life-threatening), neurological deficits (including paralysis), bowel or bladder dysfunction, transverse myelitis, cardiovascular events (e.g., myocardial infarction, stroke), discitis, osteomyelitis.
## Key Drug Interactions
* No significant drug interactions have been reported. However, concurrent use of other proteolytic enzymes is not recommended.
## Monitoring
* **Immediate Post-Procedure:** Close monitoring for signs of anaphylaxis, neurological changes, and cardiovascular events.
* **Ongoing:** Assess for pain relief, neurological status, and signs of infection.
## Clinical Pearls
* Chymopapain acts by hydrolyzing mucopolysaccharides in the nucleus pulposus, reducing disc volume and pressure on nerve roots.
* Requires administration by or under the direct supervision of a qualified physician experienced in the technique.
* All patients should be skin tested prior to administration due to the risk of anaphylaxis.
* Emergency resuscitation equipment and trained personnel must be immediately available.
