# Cefixime trihydrate
## Overview
Cefixime trihydrate is a third-generation cephalosporin antibiotic effective against a broad spectrum of Gram-negative and some Gram-positive bacteria. It is administered orally.
## Primary Indications
* Uncomplicated urinary tract infections (UTIs) caused by susceptible organisms.
* Acute otitis media.
* Pharyngitis/tonsillitis.
* Acute bronchitis and acute exacerbations of chronic bronchitis.
* Uncomplicated gonorrhea.
## Adult Dosing
* **General Dosing:** 400 mg once daily or 200 mg every 12 hours.
* **Uncomplicated Gonorrhea:** 400 mg as a single dose.
* **Maximum Dose:** 400 mg daily is generally not to be exceeded.
## Pediatric Dosing
* **General Dosing:** 8 mg/kg/day once daily or 4 mg/kg every 12 hours.
* **Maximum Dose:** 400 mg/day.
* Dosing in infants under 6 months has not been established.
## Dose Adjustments
No dose adjustment is typically required for renal impairment unless severe (CrCl < 30 mL/min). In such cases, consider reducing the dose by 25% or extending the dosing interval.
## Contraindications
* Known hypersensitivity to cefixime, other cephalosporins, penicillins, or other beta-lactams.
## Adverse Effects
* **Common:** Diarrhea, nausea, vomiting, abdominal pain, headache.
* **Serious:** *Clostridioides difficile*-associated diarrhea (CDAD), anaphylaxis, Stevens-Johnson syndrome, toxic epidermal necrolysis, seizures (rare).
## Key Drug Interactions
* **Warfarin:** Monitor INR due to potential for increased anticoagulant effect.
* **Carbamazepine:** May increase carbamazepine levels.
## Monitoring
* Monitor for signs of hypersensitivity reactions.
* Assess for effectiveness of treatment (resolution of symptoms).
* Monitor for diarrhea, especially if severe or persistent, to rule out *C. difficile*-associated diarrhea.
## Clinical Pearls
* Can be taken with or without food.
* If diarrhea is severe, consider *C. difficile*-associated diarrhea and discontinue cefixime.
* Ensure adequate hydration.
* The oral suspension should be reconstituted with the specific amount of water provided and shaken well before each use. Discard after the recommended storage period.
