# Calcium Acetate
## Overview
Calcium acetate is a phosphate binder used to reduce serum phosphorus levels in patients with hyperphosphatemia, typically associated with chronic kidney disease (CKD).
## Primary Indications
* Hyperphosphatemia in patients with chronic kidney disease.
## Adult Dosing
* **Dosage is highly individualized and titratable based on serum phosphorus levels.**
* Initiate therapy with 667 mg (10 mEq calcium) orally with meals.
* Dose should be adjusted to maintain serum phosphorus levels below 5.5 mg/dL.
* Typical doses range from 1332 mg to 4000 mg per day, divided among meals.
* **Maximum Recommended Dose:** Generally considered to be 4000 mg (approximately 2992 mg elemental calcium) per day, but may be guided by local protocol or prescriber judgment when phosphorus remains uncontrolled.
## Pediatric Dosing
* Dosing in pediatric patients is not well-established and should be managed by a specialist experienced in pediatric nephrology.
* Some sources suggest starting doses of 10-13 mEq/day divided into 3-4 doses with meals, titrating based on serum phosphorus.
## Dose Adjustments
* No specific dose adjustments are routinely required for hepatic impairment.
* Dose must be adjusted based on serum phosphorus and calcium levels.
## Contraindications
* Hypercalcemia.
* Hypersensitivity to calcium acetate.
* Concomitant use with intravenous calcium salts.
* Conditions that may lead to hypercalcemia, such as sarcoidosis, primary hyperparathyroidism.
## Adverse Effects
* **Common:** Hypercalcemia, constipation, nausea, vomiting, abdominal pain, diarrhea.
* **Serious:** Ectopic calcification, cardiovascular events (associated with hypercalcemia), renal calcification, hypophosphatemia.
## Key Drug Interactions
* **Tetracyclines and Fluoroquinolones:** Calcium acetate can bind to these antibiotics in the gastrointestinal tract, reducing their absorption. Separate administration by at least 2 hours before or 6 hours after.
* **Digoxin:** Hypercalcemia can increase the risk of digoxin toxicity. Monitor cardiac function closely.
* **Thyroid Hormones:** Calcium acetate may reduce the absorption of levothyroxine. Separate administration by at least 4 hours.
* **Antacids:** Concurrent use can increase the risk of hypercalcemia.
* **Fat-soluble vitamins:** May decrease absorption.
## Monitoring
* **Serum Phosphorus:** Monitor regularly (e.g., every 2-4 weeks initially, then every 1-3 months once stabilized) to assess efficacy and guide dosing.
* **Serum Calcium:** Monitor regularly (e.g., every 2-4 weeks initially, then every 1-3 months once stabilized). Target serum calcium levels should be maintained within the normal laboratory range or as per institutional guidelines to avoid hypercalcemia.
* **Alkaline Phosphatase:** May be monitored to assess bone disease.
* **BUN and Creatinine:** To assess renal function.
## Clinical Pearls
* Administer with meals to effectively bind dietary phosphorus.
* Calcium acetate provides more elemental calcium per dose compared to calcium carbonate, which can be advantageous when higher calcium delivery is desired for bone health postprandially.
* Be vigilant for signs and symptoms of hypercalcemia (e.g., polyuria, polydipsia, nausea, vomiting, abdominal pain, constipation, weakness, confusion, bone pain, cardiac arrhythmias).
* The goal is to control phosphorus, not to normalize calcium, but hypercalcemia is a significant risk that requires careful monitoring and dose adjustment.
* Educate patients on the importance of taking medications with meals and reporting any symptoms of hypercalcemia.
