# Calcium Acetate
## Overview
Calcium acetate is a phosphate binder used primarily in patients with end-stage renal disease (ESRD). It provides a high calcium-to-elemental-calcium ratio, facilitating lower total pill burden compared to calcium carbonate while effectively binding dietary phosphorus in the GI tract to form insoluble calcium phosphate.
## Primary Indications
Hyperphosphatemia in patients with chronic kidney disease (CKD) on dialysis.
## Adult Dosing
* **Initial Dose:** 667–1334 mg (1 to 2 tablets/capsule) orally with each meal.
* **Titration:** Adjust based on serum phosphate levels.
* **Maximum:** Generally limited by hypercalcemia risk; monitor serum calcium closely if exceeding 2000 mg elemental calcium per day from all sources.
## Pediatric Dosing
* **Safety/Efficacy:** Not well established in pediatric patients. Use is generally off-label and supervised by pediatric nephrologists.
* **Dosing:** Often extrapolated from adult data, typically 667 mg per dose taken with meals, titrated based on serum phosphate and calcium levels.
## Dose Adjustments
* **Renal Impairment:** Dose is specifically for ESRD. No renal titration required, but monitor for hypercalcemia closely as renal clearance of calcium is diminished.
## Contraindications
* Hypercalcemia.
* Hypophosphatemia.
* Renal calculi (calcium-containing).
## Adverse Effects
* **Common:** Hypercalcemia, constipation, nausea, vomiting.
* **Severe:** Metastatic calcification (due to elevated calcium-phosphorus product, generally >55 mg²/dL²), nausea, anorexia, and potential for cardiac arrhythmias if severe hypercalcemia occurs.
## Key Drug Interactions
* **Fluoroquinolones/Tetracyclines:** Calcium significantly reduces absorption. Administer at least 2 hours before or 6 hours after these antibiotics.
* **Levothyroxine:** Calcium interferes with absorption; separate doses by at least 4 hours.
* **Bisphosphonates:** Separate doses by at least 2 hours to avoid reduced absorption of the bisphosphonate.
## Monitoring
* **Serum Calcium:** Weekly during initial titration, then monthly to quarterly.
* **Serum Phosphorus:** Monthly to quarterly.
* **Calcium-Phosphorus Product:** Keep <55 mg²/dL² to minimize risks of soft tissue calcification.
## Clinical Pearls
* **Timing:** Must be taken with meals to be effective as a phosphate binder. It is ineffective if taken on an empty stomach.
* **Hypercalcemia Risk:** Calcium acetate is more soluble than calcium carbonate; assess the patient’s total daily calcium intake, including calcium-based dialysate and vitamin D analogs, to prevent iatrogenic hypercalcemia.
* **GI Side Effects:** If constipation occurs, ensure adequate fluid intake or discuss adjunct therapy.
