# Calcium Acetate
## Overview
Calcium acetate is a phosphate binder used primarily in patients with end-stage renal disease (ESRD). It is highly soluble and effectively binds dietary phosphorus in the gastrointestinal tract to form insoluble calcium phosphate for fecal excretion. It provides more elemental calcium per gram than calcium carbonate, often requiring lower total doses.
## Primary Indications
* Reduction of serum phosphorus in patients with end-stage renal disease (ESRD).
* Hypocalcemia management (less common use compared to phosphate binding).
## Adult Dosing
* **Initial Dose:** 667 mg to 1,334 mg (1 to 2 capsules/tablets) orally with each meal.
* **Titration:** Adjust based on serum phosphate levels. Most patients require 2–4 tablets per meal to achieve target phosphorus levels.
* **Maximum Dose:** Typically up to 3,000 mg–4,000 mg of elemental calcium per day; however, titration remains dependent on serum calcium and phosphate concentrations.
## Pediatric Dosing
* **Dosing:** Not well-established by labeling. Dosing is off-label and typically initiated at 333–667 mg per meal, titrated to serum phosphorus goals.
* **Note:** Always consult local pediatric nephrology protocols.
## Dose Adjustments
* **Hypercalcemia:** If hypercalcemia develops, reduce dose or discontinue therapy.
* **Renal Impairment:** No renal dose adjustment required (indicated specifically for ESRD); however, increase monitoring frequency for hypercalcemia.
## Contraindications
* Hypercalcemia.
## Adverse Effects
* **Common:** Hypercalcemia, constipation, nausea, vomiting, abdominal pain.
* **Serious:** Metastatic calcification (due to high calcium-phosphate product), cardiac arrhythmias (if severe hypercalcemia occurs).
## Key Drug Interactions
* **Fluoroquinolones/Tetracyclines:** Calcium binds these medications, significantly reducing absorption. Administer at least 2 hours before or 6 hours after calcium acetate.
* **Levothyroxine:** Calcium interferes with absorption. Administer 4 hours apart.
* **Bisphosphonates:** Calcium reduces absorption. Administer at least 2 hours apart.
* **Iron Supplements:** Concurrent use may decrease iron absorption.
## Monitoring
* **Serum Phosphorus:** Monitor regularly to ensure target levels (per KDOQI or local clinical guidelines).
* **Serum Calcium:** Monitor frequently during titration to prevent hypercalcemia.
* **Calcium-Phosphate Product:** Maintain product <55 mg²/dL² to minimize the risk of metastatic calcification.
## Clinical Pearls
* **Administration:** Must be taken **with meals** to effectively bind dietary phosphorus.
* **Calcium Content:** 667 mg of calcium acetate contains 169 mg (approx. 8.45 mEq) of elemental calcium.
* **Compliance:** The necessity of taking pills with every meal often impacts adherence; prioritize patient education on timing.
* **Vitamin D:** Evaluate concurrent Vitamin D therapy, as increased calcium absorption can lead to hypercalcemia when used with high-dose Vitamin D analogs.
