# Calcium Acetate
## Overview
Calcium acetate is a phosphate binder used to manage hyperphosphatemia in patients with end-stage renal disease (ESRD). It provides higher elemental calcium content per weight and greater phosphate-binding capacity than calcium carbonate, with less pH-dependency for dissolution.
## Primary Indications
* Reduction of serum phosphorus in patients with end-stage renal disease (ESRD).
## Adult Dosing
* **Initial Dose:** 667 mg to 1334 mg (1 to 2 capsules/tablets) orally with each meal.
* **Maintenance:** Titrate based on serum phosphate levels; often 2 to 4 capsules per meal.
* **Maximum:** Generally not to exceed 12 capsules/tablets per day unless directed by a nephrologist; clinical practice varies by local institutional protocols regarding titration goals (typically targeting serum phosphorus < 5.5 mg/dL).
## Pediatric Dosing
* **Safety/Efficacy:** Not well-established in pediatric patients.
* **Dosing:** If used, dosing is off-label and must be determined by a pediatric nephrologist based on phosphate binders’ efficacy and calcium load considerations.
## Dose Adjustments
* **Renal Impairment:** Dosing is intrinsic to ESRD management; however, reduce or hold if hypercalcemia develops (>10.5 mg/dL or >11.5 mg/dL depending on institutional guidelines).
## Contraindications
* Hypercalcemia.
* Hypercalciuria.
* Hypophosphatemia.
* Nephrolithiasis.
## Adverse Effects
* **Common:** Hypercalcemia, nausea, vomiting, constipation, abdominal pain.
* **Serious:** Metastatic calcification (vascular or soft tissue), cardiac arrhythmias.
## Key Drug Interactions
* **Tetracycline/Quinolone Antibiotics:** Decreased absorption; separate administration by at least 2 hours.
* **Levothyroxine:** Decreased absorption; separate administration by at least 4 hours.
* **Bisphosphonates:** Decreased absorption; avoid concurrent administration.
* **Calcium Channel Blockers:** Potential for decreased efficacy of the CCB due to hypercalcemia.
## Monitoring
* **Serum Phosphorus:** Monitor weekly during initiation/titration, then routinely (e.g., monthly).
* **Serum Calcium:** Monitor weekly initially; watch for signs of hypercalcemia (confusion, weakness, cardiac rhythm changes).
* **Calcium-Phosphorus Product (Ca x P):** Aim to maintain < 55 mg²/dL² to minimize risks of soft tissue calcification.
## Clinical Pearls
* **Timing:** Must be taken **with meals** to effectively bind dietary phosphorus. Taking without food does little to lower phosphate and increases the risk of hypercalcemia.
* **Elemental Calcium:** 667 mg of calcium acetate contains 169 mg (approx. 25%) of elemental calcium.
* **Hypercalcemia Risk:** Patients on concomitant Vitamin D therapy or high-calcium dialysate are at higher risk for hypercalcemia.
* **Compliance:** Frequently limited by the high pill burden; consider patient adherence when assessing efficacy.
