# Calcium Acetate
## Overview
Calcium acetate is a phosphate binder used primarily in patients with end-stage renal disease (ESRD). It is highly efficient at binding dietary phosphorus in the gastrointestinal tract to form insoluble calcium phosphate, which is then excreted in feces.
## Primary Indications
* Reduction of serum phosphorus in patients with hyperphosphatemia associated with chronic kidney disease (CKD).
## Adult Dosing
* **Initial:** 2 capsules or 1 tablet (667 mg each) with each meal.
* **Titration:** Adjust based on serum phosphate levels. Most patients require 3–4 tablets per meal to achieve target phosphorus levels.
* **Maximum:** Titrate slowly; avoid hypercalcemia.
## Pediatric Dosing
Safety and efficacy have not been established in the pediatric population. Use is strictly off-label and requires nephrology consultation; dosing is typically weight-based and titrated to serum phosphate/calcium targets.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment required for CKD; however, strictly monitor for hypercalcemia as the patient’s ability to excrete calcium is compromised.
* **Hepatic Impairment:** No dose adjustment necessary.
## Contraindications
* Hypercalcemia.
* Hypophosphatemia.
* Known hypersensitivity to calcium acetate or excipients.
## Adverse Effects
* **Common:** Nausea, constipation, diarrhea, and vomiting.
* **Serious:** Hypercalcemia (manifesting as malaise, confusion, cardiac arrhythmias, or calcification of soft tissues), metastatic calcification.
## Key Drug Interactions
* **Quinolones/Tetracyclines:** Calcium binds to these antibiotics, significantly reducing bioavailability. Separate administration by at least 2–3 hours.
* **Levothyroxine:** Calcium salts reduce absorption. Separate by at least 4 hours.
* **Bisphosphonates:** Calcium reduces oral absorption.
* **Digoxin:** Increased risk of toxicity due to potential calcium-digoxin interactions (increased cardiac sensitivity to calcium).
## Monitoring
* **Serum Phosphorus:** Monitor every 1–2 weeks during initial titration, then monthly.
* **Serum Calcium:** Monitor once or twice weekly during initial titration; ensure the Calcium-Phosphorus product (Ca x P) remains < 55 mg²/dL² to prevent metastatic calcification.
* **Signs of Hypercalcemia:** Monitor for cardiac arrhythmias and neurological status changes.
## Clinical Pearls
* **Administration:** Must be taken **with meals** to maximize phosphate binding.
* **Efficacy:** Calcium acetate is often more effective per milligram of elemental calcium than calcium carbonate because it is more soluble across the entire pH range of the GI tract.
* **Safety Warning:** Discontinue if serum calcium levels exceed 10.5–11.0 mg/dL. Ensure patients are not taking additional over-the-counter calcium supplements, as this significantly increases hypercalcemia risk.
* **Local Protocol:** Always check institution-specific guidelines regarding phosphorus targets, as they may prioritize specific Ca x P product goals per KDIGO standards.
