# Calcium acetate
## Overview
Calcium acetate is a potent phosphate-binding agent used to treat hyperphosphatemia in patients with end-stage renal disease (ESRD). It is highly soluble and provides approximately 25% elemental calcium by weight.
## Primary Indications
Hyperphosphatemia management in chronic kidney disease (CKD) patients on dialysis.
## Adult Dosing
* **Initial Dose:** 667 mg to 1,334 mg (1 to 2 capsules/tablets) orally with each meal.
* **Titration:** Adjust based on serum phosphate levels.
* **Maximum:** Typically up to 2,000 mg (elemental calcium) per day, though clinical practice often titrates to serum phosphate targets (usually < 5.5 mg/dL).
## Pediatric Dosing
* **Safety/Efficacy:** Not well-established.
* **Dosing:** If used off-label, dosing is highly variable based on institutional protocols and phosphate control; often initiated at 167–333 mg with each meal and titrated carefully to avoid hypercalcemia. Consult specialized pediatric nephrology guidelines.
## Dose Adjustments
* **Renal Impairment:** No adjustment required; standard of care for dialysis patients.
* **Hypercalcemia:** Reduce dose or suspend if serum calcium levels exceed 10.5 mg/dL or if the Calcium-Phosphorus product (Ca × P) exceeds 55 mg²/dL².
## Contraindications
* Hypercalcemia.
* Hypophosphatemia.
* Known hypersensitivity to calcium acetate or excipients.
## Adverse Effects
* **Gastrointestinal:** Nausea, constipation, diarrhea, abdominal pain (frequent).
* **Metabolic:** Hypercalcemia (especially with concomitant use of vitamin D analogs).
* **Vascular:** Potential for metastatic calcification in soft tissues if Ca × P product is chronically elevated.
## Key Drug Interactions
* **Fluoroquinolones/Tetracyclines:** Calcium significantly decreases absorption. Separate administration by at least 2 hours.
* **Levothyroxine:** Calcium interferes with absorption. Separate by at least 4 hours.
* **Bisphosphonates:** Reduces absorption.
* **Vitamin D analogs:** Concomitant use increases the risk of hypercalcemia.
## Monitoring
* **Serum Phosphate:** Monitor every 1–3 weeks during titration, then monthly to quarterly.
* **Serum Calcium:** Monitor weekly during initiation; monthly thereafter.
* **Ca × P Product:** Maintain < 55 mg²/dL² to minimize cardiovascular calcification risk.
## Clinical Pearls
* **Timing:** Must be taken with meals to be effective as a phosphate binder. It does not control phosphate if taken between meals.
* **Preference:** Often preferred over calcium carbonate in renal patients due to higher solubility across the physiological pH range of the GI tract.
* **Calcium Load:** Remind patients that this drug significantly contributes to daily calcium intake; avoid using additional calcium-based supplements unless directed.
