# Calamine Lotion
## Overview
Calamine lotion is a topical protectant and antipruritic agent usually composed of zinc oxide and ferric oxide. It functions by providing a cooling sensation upon evaporation and soothing irritated skin.
## Primary Indications
* Relief of pruritus (itch) associated with minor skin irritations (e.g., poison ivy, poison oak, poison sumac, insect bites, or chickenpox).
## Adult Dosing
* **Topical:** Apply a thin layer to the affected area 3 to 4 times daily as needed. Shake bottle well before use. There is no specific quantitative maximum dose, but usage should be limited to the affected area.
## Pediatric Dosing
* **Children (6 months and older):** Apply locally 3 to 4 times daily.
* **Infants (< 6 months):** Consult a pediatrician prior to use; avoid routine application due to potential for systemic absorption or irritation.
## Dose Adjustments
* No hepatic or renal dose adjustments required for topical application.
* Discontinue use if skin integrity is compromised or if symptoms worsen.
## Contraindications
* Hypersensitivity to zinc oxide, ferric oxide, or any component of the formulation.
* Do not apply to deep wounds, puncture wounds, or severely blistered or raw skin.
* Avoid contact with eyes, mucous membranes, or genitals.
## Adverse Effects
* **Dermatologic:** Drying of skin, mild stinging, or irritation upon application.
* **Allergic:** Contact dermatitis (rare).
## Key Drug Interactions
* No significant systemic drug-drug interactions documented for topical calamine.
* Avoid applying other topical medicated products (e.g., corticosteroids or retinoids) simultaneously to the same site unless directed by a clinician, as absorption profiles may be altered.
## Monitoring
* Monitor for signs of secondary bacterial infection (increased redness, warmth, discharge, or fever).
* Re-evaluate if condition persists or worsens after 7 days of treatment.
## Clinical Pearls
* **Shake well:** Calamine is a suspension; settling occurs naturally.
* **Drying effect:** The lotion can be drying; avoid over-application on dry or eczema-prone skin.
* **Avoid ingestion:** Calamine is for external use only. If ingested, seek medical help or contact a Poison Control Center immediately.
* **Clothing:** Calamine can stain clothing and linens; allow to dry completely after application.
* **Clinical Uncertainty:** While its antipruritic mechanism is well-established, clinical efficacy compared to modern topical antihistamines or high-potency corticosteroids is lower; it remains a safe first-line choice for minor, non-infected dermatoses.
