**Overview:**
Brimonidine is a selective alpha-adrenergic agonist used as an ophthalmic solution.
**Primary Indications:**
* Ocular hypertension
* Open-angle glaucoma
**Adult Dosing:**
* Instill 1 drop into the affected eye(s) 3 times daily.
* Maximum frequency: 3 times daily.
**Pediatric Dosing:**
* Safety and efficacy in pediatric patients have not been established. Use with caution.
**Dose Adjustments:**
* Renal impairment: Use with caution. Insufficient data to recommend specific adjustments.
* Hepatic impairment: Use with caution. Insufficient data to recommend specific adjustments.
**Contraindications:**
* Hypersensitivity to brimonidine or any component of the formulation.
* Neonates and children under 2 years of age (due to risk of CNS depression).
**Adverse Effects:**
* **Common:** Ocular redness, burning sensation, stinging, fatigue, headache, dry mouth, abnormal taste.
* **Serious:** Allergic reactions, CNS depression (especially in children), bradycardia, hypotension.
**Key Drug Interactions:**
* **MAO inhibitors:** Concomitant use is contraindicated due to potentiation of hypertensive effects.
* **Tricyclic antidepressants:** May potentiate the adrenergic effect of brimonidine.
* **CNS depressants (e.g., alcohol, barbiturates, opioids, sedatives, anesthetics):** May cause additive CNS depressant effects.
**Monitoring:**
* Intraocular pressure (IOP).
* Signs and symptoms of systemic absorption (e.g., somnolence, bradycardia, hypotension), especially in pediatric patients or those with renal/hepatic impairment.
**Clinical Pearls:**
* Brimonidine can cause systemic absorption. Exercise caution in patients with cardiovascular disease, cerebrovascular insufficiency, Raynaud's phenomenon, orthostatic hypotension, or severe depression.
* Due to its potential for CNS depression, caution is advised when operating machinery or driving.
* Consider the risk of allergic conjunctivitis; discontinue if it occurs.
