# Brimonidine
## Overview
Brimonidine is a selective alpha-2 adrenergic agonist used primarily for its ophthalmic properties.
## Primary Indications
* Glaucoma and ocular hypertension (topical ophthalmic solution)
## Adult Dosing
* **Glaucoma/Ocular Hypertension:** 1 drop instilled into the affected eye(s) three times daily.
## Pediatric Dosing
* Safety and efficacy in pediatric patients have not been established for the ophthalmic formulation.
* Systemic use (e.g., off-label oral administration) in pediatric patients carries significant risks due to potential for sedation and respiratory depression.
## Dose Adjustments
* No specific dose adjustments are typically required based on hepatic or renal impairment for ophthalmic use, as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to brimonidine.
* Use in infants and children under 2 years of age with systemic administration (e.g., oral, nasal sprays) due to risk of somnolence, coma, hypotonia, apnea, bradycardia, vomiting, and hypotension.
## Adverse Effects
* **Ophthalmic:** Allergic conjunctivitis, conjunctival hyperemia, eye dryness, foreign body sensation, blurred vision, fatigue, oral dryness, headache. Hypotension is a concern with systemic absorption.
## Key Drug Interactions
* **Antihypertensives/Diuretics:** Additive hypotensive effect may occur.
* **CNS Depressants:** Use with caution due to potential for additive CNS depressant effects (e.g., alcohol, barbiturates, opioids, sedatives, anesthetics).
## Monitoring
* Intraocular pressure (IOP).
* Signs of systemic absorption (e.g., sedation, bradycardia, hypotension) especially in pediatric patients or with prolonged or excessive use.
## Clinical Pearls
* Brimonidine has both hypotensive and pharmacologic effects via alpha-2 adrenergic agonism.
* While systemic absorption is generally low with ophthalmic use, caution is advised in patients with severe cardiovascular disease.
* Concomitant use with systemic alpha-agonists or medications affecting blood pressure should be monitored closely.
