# Brica BM (Bromhexine Hydrochloride)
## Overview
Bromhexine is a mucolytic agent that stimulates the production of serous mucus in the respiratory tract, reducing the viscosity of sputum. It also enhances the activity of ciliated epithelium and increases the clearance of bronchial secretions.
## Primary Indications
* Management of respiratory disorders associated with viscid or excessive mucus (e.g., bronchitis, COPD, asthma).
## Adult Dosing
* **Oral (Syrup/Tablet):** 8–16 mg orally three times daily.
* **Maximum Dose:** 48 mg per day.
## Pediatric Dosing
* **Children 2–6 years:** 4 mg orally twice daily.
* **Children 6–12 years:** 4 mg orally three times daily.
* **Children >12 years:** Follow adult dosing.
* *Note: Safety and efficacy in children <2 years are not well established; use only under medical supervision.*
## Dose Adjustments
* **Renal/Hepatic Impairment:** No standard dosage adjustment guidelines provided by the manufacturer; use with caution in severe hepatic or renal dysfunction due to the risk of accumulation.
## Contraindications
* Hypersensitivity to bromhexine or any component of the formulation.
* Patients with gastric ulcers (bromhexine may disrupt the gastric mucosal barrier).
## Adverse Effects
* **Common:** Nausea, vomiting, diarrhea, abdominal pain, mild transient elevation in serum transaminases.
* **Serious:** Allergic reactions (rash, urticaria, bronchospasm, angioedema), Stevens-Johnson syndrome, or Lyell’s syndrome (rare).
## Key Drug Interactions
* **Antibiotics:** Concurrent use with some antibiotics (e.g., amoxicillin, cefuroxime, erythromycin) may increase the concentration of these drugs in lung tissue; this is generally considered a therapeutic benefit rather than a negative interaction.
* **Antitussives:** Concomitant use with cough suppressants is generally discouraged as it may interfere with the physiological clearance of the liquified mucus, potentially leading to obstruction.
## Monitoring
* Monitor for worsening respiratory symptoms or signs of bronchial obstruction.
* If symptoms persist beyond 7–14 days, evaluate for underlying respiratory pathology.
## Clinical Pearls
* **Hydration:** Maintain adequate fluid intake; mucolytics are most effective when the patient is well-hydrated to facilitate mucus clearance.
* **Efficacy:** Bromhexine is often utilized as an adjunct; it does not replace the need for targeted therapy (e.g., bronchodilators or antibiotics) in patients with infection or bronchospasm.
* **Formulation:** Available as liquid, tablet, or elixir forms; check product labels for ethanol content, which may vary by brand.
