# Bilastine
## Overview
- **Classification**: Second-generation H1-antihistamine
- **Mechanism**: Selective peripheral H1-receptor antagonist. Inhibits histamine-induced wheal and flare. Minimal sedative and anticholinergic effects.
## Primary Indications
1. **Allergic Rhinitis (Seasonal & Perennial)** - Symptomatic relief of sneezing, rhinorrhea, nasal itching, congestion, and ocular symptoms.
2. **Urticaria** - Symptomatic relief of pruritus and reduction of wheal size/number.
## Adult Dosing
### Standard Dosing
**Allergic Rhinitis & Urticaria**
- **Dose**: **20 mg**
- **Frequency**: Once daily (OD)
- **Route**: Oral
- **Duration**: As needed for symptoms, continuous for chronic conditions.
### Dose Adjustments
- **Renal Impairment**: No dose adjustment required.
- **Hepatic Impairment**: No dose adjustment required.
- **Elderly Patients**: No specific dose adjustment needed.
## Pediatric Dosing
### Neonates (0-28 days)
- **Dose**: Not recommended/approved.

### Infants (1-12 months)
- **Dose**: Not recommended/approved.

### Children (6-11 years)
- **Dose**: **10 mg**

- **Special Notes**: Oral solution or orally disintegrating tablet (ODT) formulations are available. Administer on an empty stomach (1 hour before or 2 hours after food/fruit juice).
### Adolescents (12-18 years)
- **Dose**: **20 mg** (adult dose)
- **Frequency**: Once daily (OD)
- **Maximum**: **20 mg/day**
## Safety Information
### Contraindications
- **Absolute**: Hypersensitivity to bilastine or any excipients.
### Common Adverse Effects
- **Common (1-10%)**: Headache, somnolence, dizziness.
- **Less Common (0.1-1%)**: Fatigue, thirst, dry mouth, nausea, abdominal pain.
- **Serious but Rare**: Anaphylactic reaction, angioedema.
### Key Drug Interactions
- **P-glycoprotein (P-gp) Inhibitors (e.g., Ketoconazole, Erythromycin, Ritonavir, Cyclosporine)**: May increase bilastine plasma concentrations. Monitor for increased adverse effects.
- **Food/Fruit Juice (especially Grapefruit juice)**: Significantly reduces bilastine bioavailability. Administer on an empty stomach.
- **Alcohol**: Bilastine does not potentiate alcohol-induced drowsiness.
## Monitoring & Follow-up
- **Before Treatment**: No routine baseline tests required.
- **During Treatment**: Monitor for symptom relief and adverse effects.
- **Clinical Signs**: Watch for persistent drowsiness or unusual reactions.
## Clinical Pearls
- 💡 **Tip 1**: Administer **1 hour before or 2 hours after food or fruit juice** to maximize absorption.
- 💡 **Tip 2**: Generally considered **non-sedating** at recommended doses, with low potential for CNS effects.
- 💡 **Tip 3**: Does not affect the ability to drive or operate machinery at therapeutic doses.
- 💡 **Tip 4**: Effective for both seasonal and perennial allergic rhinitis, as well as chronic urticaria.
