# Arsildenfil
## Overview
Arsildenfil is a phosphodiesterase type 5 (PDE5) inhibitor. It functions by increasing blood flow to specific areas of the body.
## Primary Indications
* Erectile dysfunction (ED)
## Adult Dosing
* **Erectile Dysfunction:** 50 mg orally once daily as needed, approximately 1 hour before sexual activity.
* **Maximum Dose:** 100 mg orally once daily. Dosing should be individualized.
## Pediatric Dosing
There is no established pediatric use for Arsildenfil.
## Dose Adjustments
* **Hepatic Impairment:**
* **Mild to Moderate (Child-Pugh A or B):** Start at 25 mg orally once daily. Maximum dose is 50 mg once daily.
* **Severe (Child-Pugh C):** Contraindicated.
* **Renal Impairment:**
* **Severe (CrCl < 30 mL/min):** Start at 25 mg orally once daily. Maximum dose is 50 mg once daily.
* **Concomitant Medications:** Dosing may need to be reduced if used with certain CYP3A4 inhibitors. Consult prescribing information for specific reductions.
## Contraindications
* Concomitant use with nitrates (e.g., nitroglycerin, isosorbide mononitrate, isosorbide dinitrate) or nitric oxide donors.
* History of non-arteritic anterior ischemic optic neuropathy (NAION).
* Hypersensitivity to Arsildenfil or any of its components.
* Severe hepatic impairment.
## Adverse Effects
* **Common:** Headache, flushing, dyspepsia, visual disturbances (e.g., blurred vision, color changes), nasal congestion, back pain, myalgia.
* **Serious:** Priapism (erection lasting > 4 hours), sudden vision loss (NAION), sudden hearing decrease or loss, hypotension.
## Key Drug Interactions
* **Nitrates:** Potentially life-threatening hypotension. Avoid concomitant use.
* **Alpha-blockers:** Use with caution; may cause symptomatic hypotension. Consider dose reduction of Arsildenfil or alpha-blocker.
* **CYP3A4 Inhibitors (e.g., ketoconazole, itraconazole, ritonavir):** May increase Arsildenfil plasma concentrations. Dose reduction of Arsildenfil may be necessary.
* **Antihypertensives:** May have additive hypotensive effects.
## Monitoring
* Assess for adverse effects, particularly visual changes, hearing changes, and signs of priapism.
* Monitor blood pressure, especially in patients taking antihypertensives or alpha-blockers.
## Clinical Pearls
* Sexual stimulation is required for Arsildenfil to be effective.
* Onset of action is typically within 30-60 minutes.
* Advise patients to seek immediate medical attention for any erection lasting longer than 4 hours or sudden vision/hearing loss.
* Consider patient comorbidities and concomitant medications when selecting the initial dose.
