# Arsilodx ## Overview Arsilodx is a selective blocker of the alpha-1A adrenergic receptor. ## Primary Indications Treatment of signs and symptoms of benign prostatic hyperplasia (BPH). ## Adult Dosing * **Usual dose:** 8 mg orally once daily. * **Maximum dose:** 8 mg once daily. ## Pediatric Dosing There is no established use for Arsilodx in pediatric patients. ## Dose Adjustments * **Renal impairment:** * Mild to moderate (CrCl ≥ 30 mL/min): No dose adjustment necessary. * Severe (CrCl < 30 mL/min): Use with caution. Data is limited. * **Hepatic impairment:** * Moderate (Child-Pugh B): Start with 4 mg once daily and titrate up as tolerated. * Severe (Child-Pugh C): Contraindicated. ## Contraindications * Concomitant use with other alpha-1 adrenergic blockers. * Severe hepatic impairment. * Known hypersensitivity to silodosin. ## Adverse Effects * **Common:** Dizziness, orthostatic hypotension, headache, diarrhea, nausea, nasopharyngitis. * **Serious:** Intraoperative floppy iris syndrome (IFIS) during cataract surgery, ejaculatory disorders (in males, including retrograde ejaculation and anejaculation). ## Key Drug Interactions * **CYP3A4 Inhibitors (e.g., ketoconazole, ritonavir):** May increase silodosin plasma concentrations. Consider a reduced dose of 4 mg once daily. * **CYP3A4 Inducers (e.g., rifampin, carbamazepine):** May decrease silodosin plasma concentrations. Monitor for decreased efficacy. * **Other Alpha-1 Adrenergic Blockers:** Concomitant use is contraindicated due to risk of additive pharmacologic effects and hypotension. ## Monitoring * Blood pressure, especially after initiating therapy or increasing dose, and with concomitant use of antihypertensives. * Signs and symptoms of BPH. * Ejaculatory function. * Assess for IFIS if patient is undergoing cataract surgery. ## Clinical Pearls * Arsilodx can cause dizziness and orthostatic hypotension. Patients should be advised to sit or lie down if they experience these symptoms. * Ejaculatory disorders are common and often dose-dependent.
