# Arsil-DX (Arsil-DX / Pseudoephedrine/Chlorpheniramine/Dextromethorphan)
## Overview
Arsil-DX is a combination over-the-counter (OTC) formulation typically containing an antihistamine (Chlorpheniramine), a decongestant (Pseudoephedrine), and a cough suppressant (Dextromethorphan). It is indicated for the temporary relief of symptoms associated with the common cold, upper respiratory allergies, and sinusitis.
## Primary Indications
Symptomatic relief of nasal congestion, rhinorrhea, sneezing, and non-productive cough.
## Adult Dosing
Standard formulation: 1 tablet/dose or 10 mL every 4 to 6 hours as needed.
Maximum: Do not exceed 4 doses in 24 hours.
## Pediatric Dosing
**Children 6–12 years:** 5 mL every 4 to 6 hours as needed. Maximum 4 doses/24 hours.
**Children < 6 years:** Not recommended without direct physician supervision due to risks of severe adverse events (e.g., sedation, CNS stimulation).

## Dose Adjustments
**Renal Impairment:** Use with caution in patients with severe renal impairment due to potential accumulation of pseudoephedrine.

## Contraindications
Hypersensitivity to any component. Concurrent use or use within 14 days of MAO inhibitors. Severe hypertension or severe coronary artery disease. Narrow-angle glaucoma or urinary retention.
## Adverse Effects
- **CNS:** Drowsiness, dizziness, insomnia, tremor, anxiety.
- **Cardiovascular:** Hypertension, tachycardia, palpitations.
- **Anticholinergic:** Dry mouth, blurred vision, constipation, urinary retention.
## Key Drug Interactions
- **MAO Inhibitors:** Risk of hypertensive crisis.
- **CNS Depressants/Alcohol:** Potentiates sedative effects of chlorpheniramine.
- **Antihypertensives:** Pseudoephedrine may antagonize the efficacy of medications used to control blood pressure.
- **Serotonergic agents:** Potential risk of serotonin syndrome with dextromethorphan.
## Monitoring
Monitor blood pressure and heart rate, especially in patients with pre-existing cardiovascular conditions. Assess for urinary retention (elderly) and excessive sedation.
## Clinical Pearls
- Products labeled as "DX" or "Dextromethorphan" should not be used for chronic coughs (e.g., asthma or emphysema) or coughs accompanied by excessive phlegm.
- Patients should be counseled to avoid driving or operating heavy machinery until effect is established due to the antihistamine component.
- Confirm patient is not taking other OTC cold products to prevent accidental overdose of acetaminophen or sympathomimetics.
