# Arsilate (Arsil)
## Overview
Arsilate is a salicylate derivative, chemically related to aspirin, primarily used for its analgesic, antipyretic, and anti-inflammatory properties. It is often formulated as a sustained-release or buffered tablet.
## Primary Indications
* Management of mild to moderate pain.
* Symptomatic relief of rheumatoid arthritis, osteoarthritis, and other inflammatory conditions.
## Adult Dosing
* **Analgesic/Antipyretic:** 325 mg to 650 mg orally every 4 to 6 hours as needed.
* **Anti-inflammatory:** 650 mg to 1,000 mg orally 3 to 4 times daily.
* **Maximum Daily Dose:** Do not exceed 4,000 mg in 24 hours to avoid salicylate toxicity.
## Pediatric Dosing
* **General Use:** Not routinely recommended in children/adolescents due to the risk of Reye's syndrome (associated with salicylate use during viral infections, e.g., influenza, varicella).
* **RA/JIA:** If deemed necessary by a rheumatologist, dosing is typically 60–90 mg/kg/day divided every 6–8 hours. Use is specialized; consult pediatric rheumatology protocols.
## Dose Adjustments
* **Renal Impairment:** Exercise caution; reduced dosing may be required to prevent accumulation. Monitor creatinine clearance.
* **Hepatic Impairment:** Use with caution; monitor liver function tests (LFTs). Avoid in severe liver disease.
* **Geriatric:** Start at the lower end of the dosing range due to increased risk of GI bleeding and impaired renal clearance.
## Contraindications

* **Serious:** GI ulceration, hemorrhage, tinnitus (sign of salicylism), hepatotoxicity, renal impairment, bronchospasm.
* **Overdose:** Hyperventilation, respiratory alkalosis followed by metabolic acidosis, tinnitus, hyperthermia.
## Key Drug Interactions
* **Anticoagulants/Antiplatelets (e.g., Warfarin, Rivaroxaban):** Significantly increases risk of major bleeding.
* **NSAIDs:** Increases risk of GI toxicity and erosive gastritis.
* **Corticosteroids:** Concomitant use increases the risk of gastric ulceration.
* **Methotrexate:** Increases risk of methotrexate toxicity due to decreased renal excretion.
* **Alcohol:** Increases risk of GI bleeding.
## Monitoring
* **Long-term:** CBC (for anemia), renal function (BUN/SCr), and LFTs.
* **Safety:** Monitor for signs of GI bleeding (melena, hematemesis) and salicylism (tinnitus, vertigo, nausea, or confusion).
## Clinical Pearls
* Salicylates exhibit zero-order kinetics at higher doses, meaning small increases in dose can lead to disproportionate increases in serum concentration and risk of toxicity.
* Always advise patients to take with food or milk to minimize gastric irritation.
* Discontinue use 5–7 days prior to elective surgery due to antiplatelet effects.
* Individual protocols vary significantly; verify specific institutional guidelines for rheumatic conditions and pediatric oversight.
