# Arsil-DX (Guaifenesin / Dextromethorphan)
## Overview
Arsil-DX is a combination prescription/OTC product containing Guaifenesin (expectorant) to thin bronchial secretions and Dextromethorphan (antitussive) to suppress the cough reflex.
## Primary Indications
Symptomatic relief of cough associated with the common cold, bronchitis, or other respiratory infections accompanied by thick mucus.
## Adult Dosing
* **Dosing:** 10–20 mL (usually 200–400 mg guaifenesin / 20–40 mg dextromethorphan depending on concentration) every 4–6 hours as needed.
* **Maximum:** Do not exceed 2400 mg guaifenesin or 120 mg dextromethorphan in 24 hours.
## Pediatric Dosing
* **Children 6–12 years:** 5–10 mL every 4–6 hours as needed. Maximum: 4 doses per 24 hours.
* **Children <6 years:** Use is generally not recommended without physician oversight due to lack of evidence for efficacy and potential for toxicity.

## Dose Adjustments
* **Renal/Hepatic Impairment:** No formal adjustments established; use with caution. Guaifenesin is generally avoided in severe renal impairment (prolonged use may lead to nephrolithiasis).
## Contraindications
* Hypersensitivity to guaifenesin or dextromethorphan.
* Concomitant use with MAO inhibitors (must wait 14 days after cessation of MAOI before use) to avoid serotonin syndrome.

## Adverse Effects
* **Common:** Dizziness, drowsiness, nausea, vomiting, stomach pain.

* **MAO Inhibitors:** Risk of fatal serotonin syndrome.
* **CNS Depressants:** Increased sedation (alcohol, opioids, benzodiazepines).
* **Serotonergic Agents (SSRIs/SNRIs):** Increased risk of dextromethorphan-induced serotonin syndrome.
## Monitoring
* Monitor for symptom resolution.
* Discontinue use and seek medical attention if cough persists >7 days, is accompanied by high fever, rash, or persistent headache.
## Clinical Pearls
* **Hydration:** Instruct patients to drink plenty of water to assist the expectorant effect of guaifenesin.
* **Product Variance:** Always confirm the specific concentration (mg/mL) on the label, as "DX" products come in various strengths. Many dextromethorphan formulations contain alcohol or sugar; advise diabetic patients accordingly.
* **Abuse Potential:** Dextromethorphan is a frequent subject of substance misuse; limit pack sizes in high-risk scenarios.
