## Amoxicillin-Clavulanate Potassium
### Overview
Amoxicillin-clavulanate potassium is a combination penicillin-type antibiotic used to treat bacterial infections. It consists of amoxicillin, a beta-lactam antibiotic, and clavulanate potassium, a beta-lactamase inhibitor that prevents bacterial resistance.
### Primary Indications
* Acute bacterial otitis media
* Acute bacterial sinusitis
* Community-acquired pneumonia
* Acute exacerbations of chronic bronchitis
* Skin and skin-structure infections
* Urinary tract infections
### Adult Dosing
* **Oral:**
* **250 mg/125 mg tablets:** 1 tablet orally every 8 to 12 hours.
* **500 mg/125 mg tablets:** 1 tablet orally every 8 to 12 hours.
* **875 mg/125 mg tablets:** 1 tablet orally every 12 hours.
* **Maximum daily dose:** Generally 4000 mg amoxicillin component for oral use. Some indications may support doses up to 4000 mg/day with clavulanic acid up to 570 mg/day in divided doses every 8 hours. Precise maximums may depend on indication and local protocols.
### Pediatric Dosing
Dosing is based on the amoxicillin component and is typically calculated per kg of body weight, often with a specific mg/kg/day recommendation for mild-to-moderate infections and severe infections.
* **Suspension (e.g., 200 mg amoxicillin/28.5 mg clavulanate per 5 mL, 400 mg amoxicillin/57 mg clavulanate per 5 mL):**
* **Mild-to-moderate infections:** 25 mg/kg/day divided every 12 hours or 45 mg/kg/day divided every 8 hours.
* **Severe infections:** 45 mg/kg/day divided every 8 hours.
* **Maximum daily dose:** In pediatric patients, the maximum dose should generally not exceed the maximum adult dose for children weighing 40 kg or more. Specific maximums for the clavulanate component may also apply. Dosing is highly dependent on the concentration of the suspension and the specific product.
### Dose Adjustments
* **Renal Impairment:** Dose adjustment is required for patients with significant renal impairment. Specific recommendations for oral or IV use depend on the calculated creatinine clearance (CrCl).
* CrCl < 30 mL/min: Generally not recommended due to variable clavulanate levels. If used, consider reducing frequency.
* CrCl 10-30 mL/min: Reduce dose frequency (e.g., every 12-24 hours for 250 mg/125 mg or 500 mg/125 mg forms, every 24 hours for the 875 mg/125 mg form).
* CrCl < 10 mL/min: Reduce dose frequency (e.g., 250 mg/125 mg every 24 hours).
* **Hepatic Impairment:** Use with caution. Monitor hepatic function.
### Contraindications
* History of severe allergic reaction (e.g., anaphylaxis, Stevens-Johnson syndrome) to beta-lactams (penicillins, cephalosporins, carbapenems, monobactams).
* History of cholestatic jaundice or hepatic dysfunction associated with amoxicillin-clavulanate.
### Adverse Effects
* **Common:** Diarrhea, nausea, vomiting, rash, urticaria, pruritus.
* **Serious:** Anaphylaxis, severe skin reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis, acute generalized exanthematous pustulosis), Clostridioides difficile-associated diarrhea, hepatic dysfunction (cholestatic jaundice, hepatitis), seizures (especially in renal impairment).
### Key Drug Interactions
* **Probenecid:** May increase and prolong serum concentrations of amoxicillin and clavulanate.
* **Oral Contraceptives:** May decrease efficacy. Consider alternative or additional contraception.
* **Warfarin:** May increase INR. Monitor INR closely.
* **Allopurinol:** May increase the incidence of rash.
### Monitoring
* **Renal function:** Baseline and periodically, especially in patients with pre-existing renal disease or those at risk for renal toxicity.
* **Hepatic function:** Baseline and periodically, especially in patients with hepatic disease or those receiving prolonged therapy.
* **Signs of hypersensitivity reactions:** Monitor closely during and after therapy.
* **Signs of C. difficile infection:** Assess for persistent diarrhea.
### Clinical Pearls
* Dispersible tablets are available and can be taken with or without food.
* To minimize gastrointestinal side effects, administer at the start of a meal.
* Duration of treatment depends on the type and severity of infection.
* Ensure adequate hydration.
* Amoxicillin-clavulanate is a broad-spectrum antibiotic; judicious use is recommended to prevent resistance. It is not effective against viral infections.
