# Amoxicillin-Clavulanate (Augmentin)
## Overview
Amoxicillin-clavulanate is a combination broad-spectrum penicillin-class antibiotic and a beta-lactamase inhibitor. The clavulanic acid inhibits beta-lactamase enzymes produced by bacteria, extending the spectrum of amoxicillin to include organisms that would otherwise be resistant.
## Primary Indications
Approved for bacterial infections caused by beta-lactamase-producing organisms, including:
* Acute bacterial rhinosinusitis
* Community-acquired pneumonia
* Skin and skin structure infections
* Acute otitis media (AOM)
* Urinary tract infections (UTIs)
* Human or animal bite wounds
## Adult Dosing
Standard dosing is based on the amoxicillin component. Common formulations include 500/125 mg, 875/125 mg, or extended-release 1000/62.5 mg tablets.
* **Mild to Moderate Infections:** 500 mg amoxicillin every 8 or 12 hours.
* **Severe Infections/Respiratory Infections:** 875 mg amoxicillin every 12 hours.
* **Maximum Dose:** The 875 mg twice-daily regimen is generally considered the maximum for typical outpatient use.
## Pediatric Dosing
Dosing is based on the amoxicillin component, typically calculated at 20–45 mg/kg/day divided every 12 hours depending on infection severity.
* **Recurrent/Resistant AOM:** 80–90 mg/kg/day in two divided doses (using the 600 mg/5 mL formulation to minimize clavulanate intake and reduce diarrhea).
* **Maximum Dose:** Do not exceed adult maximums.
## Dose Adjustments
* **Renal Impairment:** Requires adjustment if CrCl <30 mL/min. Avoid the 875 mg tablet in patients with severe renal impairment (CrCl <30 mL/min).
* **Hepatic Impairment:** Use with caution; monitor liver function.
## Contraindications
* History of serious hypersensitivity (anaphylaxis) to any penicillin or cephalosporin.
* History of cholestatic jaundice or hepatic dysfunction associated with previous use of amoxicillin-clavulanate.
## Adverse Effects
* **Common:** Diarrhea (frequent due to clavulanate), nausea, vomiting, skin rash, and oral candidiasis.
* **Serious:** *Clostridioides difficile*-associated diarrhea, hypersensitivity reactions (anaphylaxis, Stevens-Johnson syndrome), and drug-induced hepatitis.
## Key Drug Interactions
* **Warfarin:** May increase INR and bleeding risk by altering gut flora; monitor INR closely.
* **Methotrexate:** May decrease clearance of methotrexate, increasing risk of toxicity.
* **BCP:** Potential reduction in efficacy of oral contraceptives (backup barrier method recommended).
* **Probenecid:** Decreases renal excretion of amoxicillin, increasing serum levels.
## Monitoring
* Baseline and periodic assessment of renal and hepatic function in long-term therapy.
* Monitor for signs of anaphylaxis during the first dose.
* Observe for signs of secondary infection (e.g., *C. difficile* diarrhea).
## Clinical Pearls
* **Administration:** Administer at the start of a meal to improve gastrointestinal tolerance and absorption.
* **Clavulanate Ratios:** Always check the ratio; formulations are not always interchangeable. Use the "ES" (extra strength) 600 mg/5 mL suspension for high-dose pediatric regimens to ensure the clavulanate content does not cause excessive diarrhea.
* **Local Protocols:** Treatment duration and first-line choices must adhere to institutional or local antibiogram-based protocols for community-acquired resistance patterns.
