# Amoxicillin-Clavulanate
## Overview
Amoxicillin-clavulanate is a combination broad-spectrum penicillin-class antibiotic. Amoxicillin provides bactericidal activity against susceptible bacteria, while clavulanate acts as a beta-lactamase inhibitor to protect amoxicillin from degradation by resistant organisms.
## Primary Indications
* Community-acquired pneumonia
* Acute bacterial rhinosinusitis
* Otitis media
* Skin and soft tissue infections (including animal/human bites)
* Urinary tract infections
## Adult Dosing
* **Standard dose:** 500 mg/125 mg orally every 8 hours or 875 mg/125 mg orally every 12 hours.
* **Severe infection/Respiratory:** 875 mg/125 mg orally every 12 hours.
* **ER/Extended Release:** 2000 mg/125 mg orally every 12 hours (must use specifically formulated ER tablets).
## Pediatric Dosing
* **Dosing is weight-based (based on amoxicillin component):** 20–45 mg/kg/day in divided doses every 8–12 hours.
* **Acute Otitis Media:** High-dose (90 mg/kg/day) often required to overcome resistant *S. pneumoniae*. Use formulations with a 7:1 or 14:1 ratio of amoxicillin to clavulanate to avoid excessive clavulanate intake (diarrhea risk).
* **Max dose:** Do not exceed adult maximums.
## Dose Adjustments
* **Renal Impairment (CrCl <30 mL/min):** Avoid the 875 mg or 2000 mg ER formulations due to high clavulanate content. Use lower-strength IR formulations and extend dosing intervals based on CrCl.
* **Hepatic Impairment:** Use with caution; monitor liver function.
## Contraindications
* History of serious hypersensitivity (anaphylaxis) to any penicillin.
* History of cholestatic jaundice or hepatic dysfunction associated with previous amoxicillin-clavulanate use.
## Adverse Effects
* **GI:** Diarrhea (common due to clavulanate), nausea, vomiting.
* **Hepatobiliary:** Elevated transaminases, cholestatic jaundice (more common in males/elderly).
* **Dermatologic:** Rash (maculopapular), especially in patients with mononucleosis.
* **Severe:** *Clostridioides difficile*-associated diarrhea, anaphylaxis.
## Key Drug Interactions
* **Methotrexate:** May decrease renal clearance, increasing systemic toxicity.
* **Warfarin:** May increase INR and bleeding risk by altering gut flora (vitamin K synthesis).
* **Oral Contraceptives:** Potential for reduced efficacy (though evidence is inconsistent; advise backup protection if diarrhea occurs).
## Monitoring
* Renal function (baseline and during prolonged therapy).
* Liver function tests (if signs of hepatotoxicity develop).
* Signs of hypersensitivity or severe diarrhea (*C. diff*).
## Clinical Pearls
* **Take with food:** Administer at the start of a meal to enhance absorption and minimize GI upset.
* **Clavulanate content:** The dose of clavulanate is the limiting factor for toxicity; ensure the chosen formulation does not exceed established daily limits.
* **Mononucleosis:** Avoid use in patients with suspected infectious mononucleosis due to a high risk of developing a non-allergic maculopapular rash.
* **Local Protocols:** Choice of dose, particularly for otitis media or community-acquired pneumonia, should follow local antibiograms and institutional guidelines.
