# Amoxicillin-Clavulanate
## Overview
Amoxicillin-clavulanate is a combination beta-lactam antibiotic consisting of an aminopenicillin (amoxicillin) and a beta-lactamase inhibitor (clavulanate). Clavulanate extends the spectrum of amoxicillin by inhibiting many plasmid-mediated beta-lactamases produced by resistant organisms.
## Primary Indications
Commonly used for acute bacterial rhinosinusitis, community-acquired pneumonia, otitis media, skin and soft tissue infections, and bite wounds (human/animal).
## Adult Dosing
* **Standard dosing:** 500 mg/125 mg orally every 8 hours or 875 mg/125 mg orally every 12 hours.
* **High-dose (e.g., severe infections):** 2000 mg/125 mg (extended-release formulations) orally every 12 hours.
* **Note:** Dosing varies significantly by indication and local resistance patterns; always reference institutional or national guidelines (e.g., IDSA).
## Pediatric Dosing
* **Based on Amoxicillin component:** 20 to 45 mg/kg/day in divided doses every 8–12 hours.
* **Otitis Media/Severe Infections:** Often 80–90 mg/kg/day (using 4:1 or 7:1 ratio formulations to avoid excessive clavulanate) divided every 12 hours.
* **Limit:** Do not exceed adult dosing.
## Dose Adjustments
* **Renal Impairment:** Requires dose reduction or longer interval for CrCl <30 mL/min. Do not use the 875 mg tablet in patients with severe renal impairment (CrCl <30 mL/min).
* **Hepatic Impairment:** Use with caution; monitor liver function.
## Contraindications
* History of serious hypersensitivity reactions (anaphylaxis) to any penicillin.
* History of cholestatic jaundice or hepatic dysfunction associated with previous amoxicillin-clavulanate use.
## Adverse Effects
* **Gastrointestinal:** Diarrhea (very common, related to clavulanate), nausea, vomiting, abdominal pain.
* **Hepatic:** Elevated transaminases; rare but serious drug-induced hepatitis/cholestatic jaundice (more common in males >60 years or with prolonged therapy).
* **Other:** Rash (maculopapular), candidiasis (oral/vaginal).
## Key Drug Interactions
* **Methotrexate:** May decrease clearance of methotrexate, increasing risk of toxicity.
* **Warfarin:** May increase INR/risk of bleeding; monitor PT/INR closely when initiating or stopping therapy.
* **Oral Contraceptives:** Potential for reduced efficacy (though evidence is debated, backup contraception is often recommended during use).
## Monitoring
* **Renal:** Periodic assessment of renal function in prolonged therapy.
* **Hepatic:** Monitor for signs of jaundice or elevated liver enzymes if therapy exceeds 10–14 days.
* **General:** Monitor for signs of anaphylaxis during the first dose and for *Clostridioides difficile*-associated diarrhea.
## Clinical Pearls
* **The Clavulanate Ratio:** Use lower-ratio formulations when possible (or specific pediatric formulations) to minimize the risk of diarrhea caused by high-dose clavulanate.
* **Food:** Administer at the start of a meal to improve absorption and reduce GI side effects.
* **Do Not Interchange:** Extended-release formulations (e.g., 2000 mg) are not therapeutically equivalent to two 1000 mg tablets due to pharmacokinetic differences.
* **Allergy Cross-reactivity:** Use with extreme caution in patients with history of IgE-mediated reactions to cephalosporins.
