# Amoxicillin-Clavulanate
## Overview
Amoxicillin-clavulanate is a combination aminopenicillin antibiotic and beta-lactamase inhibitor. The addition of clavulanic acid extends the spectrum of activity to include beta-lactamase-producing strains of *S. aureus*, *H. influenzae*, *M. catarrhalis*, and certain anaerobes.
## Primary Indications
Commonly used for acute bacterial rhinosinusitis, community-acquired pneumonia, animal/human bites, skin and soft tissue infections, and urinary tract infections.
## Adult Dosing
Standard dosing is typically based on the amoxicillin component.
* **Mild to moderate severity:** 500 mg orally every 8 hours or 875 mg orally every 12 hours.
* **Severe infections:** 875 mg orally every 8 hours (using extended-release formulations or multiple tablets).
* **Note:** Due to the clavulanate content, do not use two 875 mg tablets to achieve a higher dose; this results in excessive clavulanate, which may cause GI distress.
## Pediatric Dosing
**Dosing should be based on weight and the specific formulation ratio.** Pediatric formulations come in various ratios (e.g., 4:1, 7:1); always verify the ratio on the prescription.
* **Standard dose:** 25–45 mg/kg/day (based on amoxicillin component) divided every 12 hours.
* **Severe/Recurrent infections (e.g., Acute Otitis Media):** 80–90 mg/kg/day divided every 12 hours (often using the 600 mg/5 mL formulation to minimize clavulanate exposure).
* **Maximum:** Follow specific age/weight-based recommendations; generally capped at adult maximums.
## Dose Adjustments
* **Renal Impairment:** If CrCl < 30 mL/min, the use of extended-release formulations is generally contraindicated. Reduce frequency or dose based on CrCl level; avoid specific formulations (e.g., 875 mg) in severe renal impairment.
* **Hepatic Impairment:** Use with caution; monitor hepatic function.
## Contraindications
* History of serious hypersensitivity (anaphylaxis) to any penicillin.
* History of cholestatic jaundice or hepatic dysfunction associated with previous use of amoxicillin-clavulanate.
## Adverse Effects
* **Gastrointestinal:** Diarrhea (very common), nausea, and vomiting. Administer with food to minimize GI upset.
* **Hepatic:** Potential for drug-induced hepatitis or cholestatic jaundice (more common in males and the elderly).
* **Dermatologic:** Rash (specifically non-allergic maculopapular rash, often seen in EBV infections).
* **Other:** Superinfection (C. difficile-associated diarrhea, candidiasis).
## Key Drug Interactions
* **Warfarin:** May increase INR and bleeding risk; monitor INR closely.
* **Methotrexate:** May decrease clearance of methotrexate, increasing toxicity.
* **Oral Contraceptives:** May reduce efficacy; suggest backup contraception during therapy.
## Monitoring
* **Signs of hypersensitivity:** Anaphylaxis or severe skin reactions.
* **Renal/Hepatic:** Periodic baseline and follow-up LFTs and renal function tests if long-term therapy is required.
* **GI:** Monitor for severe diarrhea or persistent bloody stools (potential *C. difficile*).
## Clinical Pearls
* **Clavulanate Ratio:** Use the 600 mg/5 mL oral suspension for high-dose pediatric therapy to reduce the total amount of clavulanate administered, which significantly reduces the incidence of diarrhea.
* **Food:** Always advise patients to take the medication at the start of a meal to improve absorption and reduce GI side effects.
* **Allergy:** A history of a mild "rash" to penicillin in childhood may not be a true IgE-mediated allergy; consider referral for penicillin allergy testing if the history is ambiguous.
