# Amorolfine Nail Lacquer
## Overview
Amorolfine is a topical antifungal agent used for the treatment of fungal nail infections (onychomycosis). It works by inhibiting the synthesis of ergosterol, an essential component of fungal cell membranes.
## Primary Indications
Fungal infections of the nails (onychomycosis). It is generally indicated for mild to moderate cases involving up to two-thirds of the nail plate and fewer than four nails.
## Adult Dosing
Apply thinly to the affected nail(s) once or twice weekly, as directed by the manufacturer or local protocol.
* **Application:** File down the infected areas of the nail(s) thoroughly with a nail file. Clean and dry the nail(s). Apply the lacquer evenly over the entire nail surface. Allow to dry completely.
* **Duration:** Treatment duration typically ranges from 6 months for fingernails and 9-12 months for toenails, or until the infection has cleared and healthy nail has regrown.
## Pediatric Dosing
Safety and efficacy in pediatric patients have not been established. Use is generally not recommended.
## Dose Adjustments
No specific dose adjustments are typically required for renal or hepatic impairment due to its minimal systemic absorption.
## Contraindications
* Hypersensitivity to amorolfine or any component of the formulation.
* Pregnancy and breastfeeding (use with caution, benefits should outweigh risks).
## Adverse Effects
* **Local:** Nail discoloration, nail damage, nail brittleness, application site reactions (burning, itching, redness).
* **Systemic:** Very rare, but hypersensitivity reactions can occur.
## Key Drug Interactions
* **Other topical nail treatments:** Avoid concurrent use of other topical nail preparations unless specifically advised by a healthcare professional.
* **Systemic antifungals:** No significant interactions are expected due to minimal systemic absorption.
## Monitoring
* Monitor for signs of clinical improvement (e.g., reduction in fungal load, regrowth of healthy nail).
* Assess for and manage any local adverse reactions.
## Clinical Pearls
* Consistent application as prescribed is crucial for treatment success.
* Proper nail preparation (filing and cleaning) before application enhances efficacy.
* Avoid contact with mucous membranes.
* Wash hands thoroughly after application.
