# Amorolfine Nail Lacquer
## Overview
Amorolfine is a morpholine-derivative topical antifungal agent. It exerts fungistatic and fungicidal activity by inhibiting delta-14-reductase and delta-7,8-isomerase, depleting ergosterol and causing accumulation of atypical sterols in the fungal cell membrane.
## Primary Indications
Treatment of mild distal/lateral subungual onychomycosis caused by dermatophytes, yeasts, and molds (when matrix is not involved and ≤2 nails are affected).
## Adult Dosing
Apply to the affected fingernails or toenails once or twice weekly.
* **Duration:** Treat fingernails for 6 months; treat toenails for 9–12 months.
* **Maximum:** Generally limited to 2 nails per product license; efficacy decreases if infection involves the lunula (matrix).
## Pediatric Dosing
Safety and efficacy have not been established in children. Use is generally not recommended in pediatric patients unless under the guidance of a specialist.
## Dose Adjustments
No specific dosage adjustments are required for elderly patients or patients with renal/hepatic impairment, as systemic absorption is negligible.
## Contraindications
* Hypersensitivity to any component of the lacquer.
* Do not apply to infected skin surrounding the nail, inflamed tissue, or open wounds.
## Adverse Effects
* **Common:** Temporary burning sensation, erythema, or irritation of the periungual skin.
* **Rare:** Contact dermatitis, nail discoloration, or brittle/breaking nails.
## Key Drug Interactions
No clinically significant drug-drug interactions have been identified due to low systemic absorption. Users should avoid applying cosmetic nail varnish or artificial nails during treatment.
## Monitoring
* Monitor for clinical improvement every 3 months.
* Assess for local skin irritation or allergic reaction at the application site.
* Therapy should be discontinued if no improvement is noted after 6–9 months.
## Clinical Pearls
* **Preparation:** File down the infected areas of the nail (including the surface) as much as possible before each application. Clean the nail surface with an alcohol swab provided in the kit.
* **Application:** Apply the lacquer to the entire nail surface using the provided applicator. Allow to dry completely (3–5 minutes).
* **Hygiene:** Use a new file for each treatment session to avoid cross-contamination. Files used for infected nails must not be used on healthy nails.
* **Efficacy:** Success rates are significantly lower than oral systemic therapies; this agent is best reserved for mild cases without matricial involvement.
