# Amorolfine Nail Lacquer
## Overview
Amorolfine is a morpholine-derivative antifungal agent. It disrupts fungal cell membrane synthesis by inhibiting delta-14-reductase and delta-7,8-isomerase, leading to depletion of ergosterol and accumulation of unnatural sterols. It is formulated as a topical lacquer (typically 5%) for the treatment of onychomycosis.
## Primary Indications
Treatment of fungal infections of the nails (onychomycosis) caused by dermatophytes, yeasts, and molds.
## Adult Dosing
Apply to the affected fingernails or toenails once or twice weekly, according to local medical advice. Treatment should continue until the infected nail is completely regenerated and the affected areas are cured (typically 6 months for fingernails and 9–12 months for toenails).
## Pediatric Dosing
Safety and efficacy in children have not been established. Use is generally not recommended in pediatric patients unless directed by a specialist.
## Dose Adjustments
No specific dosage adjustments are required for renal or hepatic impairment due to negligible systemic absorption.
## Contraindications
Hypersensitivity to amorolfine or any component of the lacquer.
## Adverse Effects
- **Local:** Burning sensation, erythema, pruritus, or contact dermatitis at the application site.
- **Nail changes:** Rare reports of nail discoloration, nail plate fragility, or onychoclasis (breaking of the nail).
## Key Drug Interactions
There are no clinically significant systemic drug interactions reported due to minimal transdermal absorption. Topical application of other medicated nail products on the same nail should be avoided.
## Monitoring
- Monitor for signs of local irritation or hypersensitivity.
- Evaluate progress; if no improvement is observed after 6–12 months, consider alternative antifungal therapy (e.g., systemic agents).
- Patients should be monitored for the development of secondary bacterial infections (paronychia).
## Clinical Pearls
- **Application Technique:** Before the first application, file down the affected area of the nail (including the surface) as thoroughly as possible using the provided nail file. Clean the surface with an alcohol swab.
- **Hygiene:** Files used on infected nails must not be used on healthy nails.
- **Obstruction:** Avoid the use of artificial nails or nail polish during treatment with amorolfine lacquer.
- **Protection:** Wear impermeable rubber or plastic gloves when handling organic solvents (e.g., paint thinner, white spirit) to prevent the lacquer from dissolving.
