# Amorolfine Nail Lacquer
## Overview
Amorolfine is a morpholine-derivative topical antifungal agent. Unlike other antifungals, it inhibits the enzyme delta-14-reductase and delta-7,8-isomerase, which depletes ergosterol and causes the accumulation of atypical sterols in the fungal cell membrane, leading to fungal cell death.
## Primary Indications
Treatment of distal subungual onychomycosis caused by dermatophytes, yeasts, and molds.
## Adult Dosing
Apply to the affected fingernails or toenails once or twice weekly.
1. File down the affected areas of the nail (including the nail surface) as thoroughly as possible using the provided nail file.
2. Clean the surface of the infected nail using an alcohol swab.
3. Apply the lacquer evenly over the entire surface of the infected nail using the provided spatula.
4. Allow to dry for 3–5 minutes.
5. Duration: Fingernails typically require 6 months and toenails 9–12 months of continuous treatment.
## Pediatric Dosing
Safety and efficacy in children have not been established. Use is generally not recommended in pediatric patients unless directed by a specialist.
## Dose Adjustments
No specific dosage adjustments are required for renal or hepatic impairment due to negligible systemic absorption.
## Contraindications
Hypersensitivity to amorolfine or any component of the formulation.
## Adverse Effects
- Localized skin irritation (burning, erythema, pruritus) around the nail bed.
- Nail disorders (discoloration, brittle or broken nails) are rare but reported.
- Contact dermatitis.
## Key Drug Interactions
No clinically significant systemic drug interactions are known due to minimal systemic absorption. Avoid applying other nail lacquers or cosmetic nail products to the treated nail during therapy.
## Monitoring
- Examine the nails every 3 months to assess clinical progress.
- Monitor for signs of local inflammation or hypersensitivity.
- If no improvement is seen after 6–12 months, reassess the diagnosis or treatment modality.
## Clinical Pearls
- **File usage:** Files used on infected nails must not be used on healthy nails to prevent the spread of infection.
- **Occlusion:** Do not apply artificial nails or nail polish during the treatment course.
- **Chemical exposure:** Wear impermeable gloves when using organic solvents (e.g., thinners, white spirit) to prevent the lacquer from dissolving.
- **Efficacy:** If the infection is severe or involves the nail matrix (lunula), topical amorolfine monotherapy is often insufficient, and systemic oral antifungals (e.g., terbinafine) may be required.
