# Amorolfine nail lacquer
## Overview
Amorolfine is a morpholine-derivative topical antifungal agent that acts by inhibiting sterol $\Delta^{8} \to \Delta^{7}$ isomerase and $\Delta^{14}$ reductase, depleting ergosterol and causing accumulation of atypical sterols in the fungal cell membrane.
## Primary Indications
Treatment of fungal nail infections (onychomycosis) caused by dermatophytes, yeasts, and molds.
## Adult Dosing
Apply to the affected fingernails or toenails once or twice weekly. Treatment should continue until the affected nail has regenerated and the lesion is completely healed.
* **Duration:** Typically 6 months for fingernails and 9–12 months for toenails.
* **Maximum:** No specific quantitative maximum, but systemic absorption is negligible.
## Pediatric Dosing
Safety and efficacy have not been established in children. Professional judgment or local protocols usually discourage use in pediatric populations unless under direct specialist supervision.
## Dose Adjustments
No specific dosage adjustments are required for renal or hepatic impairment due to minimal systemic absorption.
## Contraindications
* Hypersensitivity to amorolfine or any product excipients.
* Application on skin surrounding the nail if inflammation or lesions are present.
## Adverse Effects
* **Common:** Mild, transient periungual skin irritation, burning sensation, erythema, scaling, or contact dermatitis.
* **Rare:** Nail discoloration, brittle or broken nails.
## Key Drug Interactions
No clinically significant systemic drug interactions are recognized due to minimal systemic absorption. Avoid applying other topical medicaments to the same nail during treatment.
## Monitoring
Monitor the nail for regrowth of healthy tissue. Review treatment efficacy after 3–6 months. If no improvement is noted, consider adjunctive oral antifungal therapy.
## Clinical Pearls
* **Application Technique:** Before the first application, file down the infected parts of the nail (including the surface) as thoroughly as possible. Clean and degrease the nail surface using an alcohol swab before applying the lacquer.
* **Hygiene:** Use the provided spatula to apply the lacquer; clean after each use with a swab soaked in alcohol. Do not use the same file for healthy nails and infected nails.
* **Cosmetic Use:** Avoid using artificial nails or nail polish during the course of treatment.
* **Protection:** Wear impermeable gloves when working with organic solvents (thinners, white spirit) to prevent dissolving the lacquer.
