# Amorolfine Nail Lacquer
## Overview
Amorolfine is a morpholine-derivative antifungal agent. It exerts fungistatic and fungicidal activity by inhibiting $\Delta^{14}$-reductase and $\Delta^{7-8}$-isomerase, enzymes essential for the biosynthesis of ergosterol in the fungal cytoplasmic membrane.
## Primary Indications
Treatment of distal and lateral subungual onychomycosis caused by dermatophytes, yeasts, and molds, where the nail matrix is not involved.
## Adult Dosing
Apply to the affected fingernails or toenails **once weekly**.
* **Duration:** Continue until the infected nail is completely regenerated and the affected areas are cured. Treatment typically lasts 6 months for fingernails and 9–12 months for toenails.
* **Maximum:** No maximum cumulative dose, but restricted to the quantity required to cover diseased nail surface area.
## Pediatric Dosing
Safety and efficacy have not been established in children. Use in pediatric patients is generally off-label and requires clinical judgment; dose follows the adult application frequency if used.
## Dose Adjustments
* **Renal/Hepatic Impairment:** No dose adjustments are required due to minimal systemic absorption.
* **Geriatric:** No dosage adjustment necessary.
## Contraindications
* Hypersensitivity to amorolfine or any components of the lacquer.
## Adverse Effects
* **Common:** Temporary burning sensation, slight erythema, or transient irritation of the skin surrounding the nail.
* **Rare:** Contact dermatitis, nail disorders (discoloration, brittle nails, or onychoclasis).
## Key Drug Interactions
No clinically significant drug interactions have been reported due to negligible systemic absorption. Avoid applying other nail lacquers or cosmetic nail products to treated nails.
## Monitoring
* Monitor for clinical improvement every 3 months.
* In patients with peripheral vascular disease, diabetes, or immune system disorders, monitor the site frequently for signs of secondary infection or worsening conditions.
## Clinical Pearls
* **Preparation:** Before the first application, file down the infected parts of the nail (including the surface) as thoroughly as possible using the provided nail file. The surface must be cleaned and degreased with an alcohol pad before applying medication.
* **Application:** Allow the lacquer to dry for 3–5 minutes. If the lacquer is reapplied, all previous layers must be removed with alcohol and the nail surface filed again.
* **Hygiene:** Files used for infected nails should not be used on healthy nails to prevent cross-contamination.
* **Chemical Exposure:** Wear impermeable gloves when handling organic solvents (e.g., detergents, thinners) to prevent the lacquer from dissolving.
